Expression of key signaling molecules in PI3K/AKT/MTOR signaling pathway in gastric cancer tissues and the association with clinical manifestations

摘要 目的探讨磷脂肌醇3-激酶(PI3K)/蛋白激酶B(AKT)/雷帕霉素靶蛋白(MTOR)信号通路关键信号分子在胃癌组织、癌旁组织及正常胃黏膜组织中的表达情况。方法收集120例同一患者胃癌组织、癌旁组织及正常胃黏膜组织标本,采用免疫组化法及Western blot法检测PI3K/AKT/MTOR信号通路的关键信号分子p-AKT及p-MTOR在3组标本中的表达情况,并分析120例胃癌患者p-AKT及p-MTOR表达与临床特征的关系。结果胃癌组织中p-AKT阳性表达率为71.67%,分别高于癌旁组织(30.00%)及正常胃黏膜组织(24.17%),差异均有统计学意义(P﹤0.05);癌旁组织及正常胃黏膜组织中p-AKT阳性表达率比较,差异无统计学意义(P﹥0.05)。胃癌组织、癌旁组织中p-MTOR阳性表达率分别为76.67%和64.00%,均高于正常胃黏膜组织(19.17%),差异均有统计学意义(P﹤0.05);胃癌组织、癌旁组织中p-MTOR阳性表达率比较,差异无统计学意义(P﹥0.05)。p-AKT、p-MTOR在低分化、TNM分期为Ⅲ+Ⅳ期的胃癌组织中阳性表达率及蛋白相对表达量均高于高中分化、TNM分期为Ⅰ+Ⅱ期的胃癌组织,差异均有统计学意义(P﹤0.05);而不同年龄、肿瘤直径、浸润深度、淋巴结转移情况胃癌患者的胃癌组织中p-AKT、p-MTOR阳性表达率及蛋白相对表达量比较,差异均无统计学意义(P﹥0.05)。结论 PI3K/AKT/MTOR信号通路的关键信号分子异常活化与过度表达,对胃癌的发生、发展可能起着促进作用,在开辟生物治疗途径上具有重要临床应用价值。 Objective To investigate the expression of the key signaling molecules phosphoinositide-3 kinase (PI3K) / protein kinase B (AKT) / mechanistic target of rapamycin kinase (MTOR) in gastric cancer tissues, adjacent normal tissues and normal gastric mucosal tissues.Method There were 120 cases included in the analysis, from which the gastric cancer tissues, adjacent normal tissues and normal gastric mucosal tissues were collected, and immunohistochemical assay and Western blot staining were used to detect the expression of key signaling molecules p-AKT and p-MTOR in PI3K/ AKT/MTOR signaling pathways, besides, 120 patients with gastric cancer was analyzed for the relationship between p- AKT and p-MTOR expression and clinical manifestations. Result The positive expression of p-AKT in gastric cancer tissues was 71.67%, which was higher than that in adjacent normal tissues (30.00%) and normal gastric mucosal tissues (24.17%), with statistically significant difference observed (P〈0.05); while that was comparable between adjacent normal tissues and normal gastric mucosal tissues (P〉0.05). In gastric cancer tissues and adjacent normal tissues, the positive expression rate of p-MTOR was 76.67% and 64.00%, respectively, and were higher than that in normal gastric mucosal tissues at 19.17%, showing statistically significant difference (P〈0.05); and p-MTOR was similarly expressed in gastric cancer tissues and adjacent normal tissues (P〉0.05). Higher positive expression rate of p-AKT and p-MTOR were observed in gastric cancer tissues with poor differentiation and TNM stage of III+IV than in those with high differentiation and TNM stage of I+II, showing statistically significant difference (P〈0.05); while in gastric cancer tissues from patients with different age, tumor diameter, depth of invasion and lymph node metastasis, the positive expression rate and relative protein expression level were of no statistically significant difference (P〉0.05).Conclusion The abnormal activation and overexpression of the key signaling molecules in PI3K/AKT/MTOR signaling pathway is associated with the occurrence and development of gastric cancer, and is clinically meaningful for exploring novel biological approach for the disease.
