摘要
目的：观察中西医结合周期治疗对卵泡发育不良不孕症患者促排卵加指导同房后妊娠结局的影响。方法：将符合纳入标准的不孕症患者共100例,随机分为对照组40例和研究组60例,对照组（40例）自月经或撤退性出血的3～5 d予以口服克罗米芬,排卵期阴道彩超监测排卵至18 mm,予注射绒毛膜促性腺激素（HCG）8000～10 000 IU,嘱患者注射HCG后1、2 d同房;黄体期予以地屈孕酮黄体支持。研究组（60例）在对照组治疗的基础上,卵泡期、黄体期分别予以中药周期治疗。连续观察1～3个周期。治疗结束后比较两组患者最大卵泡直径、排卵率、排卵期子宫内膜厚度及妊娠率,评价临床疗效。结果：经过1～3个周期治疗,对照组与研究组有效率均有明显提高,且研究组有效率提高更明显;研究组和对照组临床妊娠率分别为38.33%和20.0%,差异有统计学意义（P〈0.05）。结论：中西医结合治疗卵泡发育不良不孕症的疗效优于单用西药组。中西医结合周期序贯治疗弥补了应用克罗米芬（CC）所致子宫内膜变薄的缺点,增加子宫内膜的容受性,从而提高妊娠率。
Objective：To observe the effect of traditional Chinese and Western medicine combined with cycle therapy on the pregnancy outcome after ovulation induction and guidanced intercourse in infertile women with follicular maldevelopment. Methods：A total of 100 infertile patients who met the inclusion criteria were randomly divided into control group（n=40） and study group（n=60）. The control group（40 cases） after menstruation or withdrawal bleeding 3-5 days was treated with clomiphene. Ovulation color transvaginal ultrasound monitored ovulation to 18 mm. HCG 8000-10 000 IU was injected. After 1 and 2 days of injection,the patients could intercourse. Luteinium luteum was supported by luteinium progesterone. The study group（60 cases） was treated with traditional Chinese medicine cycle in the follicular phase and luteal phase on the basis of the treatment of the control group. We observed 1-3 cycles. After treatment, the maximum follicle diameter, ovulation rate, endometrial thickness and pregnancy rate in the two groups were compared and the clinical efficacy was evaluated. Results：After 1-3 treatment cycles,two groups＇ effective rats were increased and the study group＇s was more obvious. The pregnancy rates of the study group and control group were 38.33% and 20.0%,respectively（P〈0.05）. Conclusion：The curative effect of integrated traditional Chinese and Western medicine in the treatment of infertility with follicular maldevelopment is better than that of Western medicine alone. The sequential treatment of traditional Chinese and Western medicine can make up for the weakness of endometrial thinning caused by clomiphene（CC） increasing the endometrial receptivity, so as to improve the pregnancy rate.
