摘要
目的 分析强的松联合他克莫司治疗对狼疮性肾炎患者血浆T淋巴细胞亚群和肾功能的影响.方法 随机选取2013年7月至2016年7月期间在本院就诊的难治性狼疮性肾炎患者384例为研究对象,分为实验组（强的松＋他克莫司）和对照组（强的松＋环磷酰胺）,各192例,分别计算患者血浆T淋巴细胞亚群中CD3＋、CD3＋ CD4＋、CD3＋ CD8＋和CD20＋细胞在外周血淋巴细胞中的百分率与计数,观察两组患者治疗前和治疗3个月后的24h尿蛋白、血清白蛋白与血肌酐水平,采用SPSS 20.0统计学软件进行分析.结果 实验组的CD3＋、CD4＋、CD8＋、CD4＋CD25＋ Foxp3＋细胞数高于对照组;实验组的总疗效高于对照组;治疗后,实验组的24h尿蛋白含量低于对照组,实验组的血清蛋白含量高于对照组,两组之间比较有统计学差异（P＜0.05）.结论 强的松联合他克莫司治疗狼疮性肾炎患者后,血浆T淋巴细胞亚群中CD3＋、CD4＋和CD8＋细胞数增加,总疗效提高了,24h尿蛋白含量降低,血清蛋白含量升高,肾功能得到完善.
Objective To study the effect of prednisone combined with tacrolimus therapy on T lymphocyte subsets and renal function in patients with lupus nephritis.Methods A total of 384 patients with lupus nephritis from July 2013 to July 2016 in our hospital were randomly selected and divided into experimental group （tacrolimus with prednisone） and control group （prednisone with cyclophosphamide）,192 cases each group.The percentage and count were calculated in patients with T lymph cell subsets in CD3 ＋,CD3 ＋ CI4 ＋,CD3 ＋ CD8 ＋ and CD20 ＋ cells in peripheral blood lymphocytes of the patients.Before treatment and after 3 months,the 24 h urine protein,serum albumin and serum creatinine level of two groups were observed and analyzed by SPSS 20.0 statistical software.Results The number ofCD3＋,CI4＋,CD8＋,CD4＋CD25＋Foxp3＋cells in the experimental group was higher than that in the control group.The total effective rate of the test group was higher than that of the control group.After 24 hours,the urinary protein content of the test group was lower than the control group （P 〈 0.05）,and there was significant difference between the two groups （P 〈 0.05）.Conclusions Prednisone combined with tacrolimus in the treatment of patients with lupus nephritis,the plasma levels of T lymphocyte subsets of CD3 ＋,CD4 ＋ and CD8 ＋ cell number increase,the curative effect improves,and content of 24 h urine protein decreases,serum protein and renal function improve.
