摘要
目的探讨全麻下手法松解肩关节配合术后康复锻炼治疗肩周炎的临床疗效。方法将40例肩周炎患者完全随机分为治疗组20例,对照组20例。治疗组采用全麻下手法松解1次后,配合康复锻炼治疗3周,对照组采用单纯康复锻炼治疗3周。根据视觉模拟评分法(VAS)观察2组治疗前后患肩的疼痛症状改善情况,并通过主动关节活动度(AROM)对治疗前后的患肩活动功能进行评价。结果治疗1周、3周时,2组患肩的VAS疼痛评分、AROM评分均优于治疗前(P<0.05)。治疗3周时,治疗组的VAS疼痛评分、AROM评分均优于治疗1周时(P<0.05);对照组的VAS疼痛评分较治疗1周时无明显差异(P>0.05),AROM评分优于治疗1周时(P<0.05)。治疗1周、3周时,治疗组的VAS疼痛评分、AROM评分均优于对照组,差异有统计学意义(P<0.05)。结论全麻下手法松解配合康复锻炼治疗肩周炎的短期临床疗效优于单纯康复锻炼。
Objective Explore the clinical effect of the postoperative rehabilitation exercise on shoulder periarthritis after general anesthesia. Methods 40 patients with periarthritis of shoulder were randomly divided into treatment group（n = 20） and control group（n = 20）. In the treatment group, the treatment group was treated with general anesthesia for 3 weeks, and the control group was treated with simple rehabilitation exercise for 3 weeks.Visual simulation score（VAS） was used to observe the improvement of shoulder pain symptoms before and after treatment, and to evaluate the function of shoulder activity before and after treatment by active joint activity（AROM）. Results The VAS pain score and AROM score of the two groups were better than those before treatment（P〈0.05）.The VAS pain score and AROM score of the treatment group were better than that of 1 week（P〈0.05）. The VAS pain score in the control group showed no significant diff erence（P〉0.05） compared with the treatment of 1 week, and the AROM score was better than that of treatment 1 week（P〈0.05）.The VAS pain score and AROM score in the treatment group were better than those in the control group for 1 week and 3 weeks, with statistically significant difference（P〈0.05）. Conclusion The short-term clinical curative effect of general anesthesia combined with rehabilitation exercise for shoulder periarthritis is better than simple rehabilitation exercise.
