摘要目的探讨临床药师参与肾脏替代治疗患者抗菌药物剂量调整的药学实践,为临床合理用药提供参考。方法临床药师参与1例肾脏替代治疗继发腹膜炎患者抗感染方案的制定,并对3种肾脏替代治疗方案转换时抗菌药物剂量的调整提出合理建议。结果患者抗感染治疗取得良好效果,治疗过程中无药品不良事件发生,病情稳定出院。结论肾脏替代治疗患者常合并感染,且有不同治疗方式转换,临床药师应利用专业知识,协助医师改善用药策略并优化剂量方案,保障患者用药安全。
