摘要
目的探讨失效模式与效应分析(Fail-ure ModeandEffectAnalysis,FMEA)在预防与控制导管室医院感染中的应用效果。方法运用FMEA方法,对导管室医院感染风险进行评估,针对高风险因素,使用根本原因分析法(root cause analysis,RCA)进行原因分析,制定并实施针对性的防范措施,比较FMEA前后的效果。结果改进后医院感染发生率由0. 34%降低至0,医务人员职业暴露发生率由45. 45%下降至11. 11%(P <0. 05,数据有统计学意义);优先风险系数(RPN)计算显示,风险水平由高、中下降至低。结论 FMEA方法可有效降低导管室医院感染风险,防范介入手术相关医院感染的发生。
Objective To explore the effect of failure mode and effeet analysis （FMEA） on the prevention and control of nosocomial infection in the catheter room. Methods The risk of nosoeomial infection in catheter room was evaluated by FMEA method, and the high risk factors were analyzed by root cause analysis（RCA） method. Targeted preventive measures were formated and implemented. The effects before and alter FMEA were compared. Results Alter the application of FMEA, the incidence of nosoeomial infection decreased from 0.34% to 0% , and the occupational exposure rate of medical staff decreased from 45.45% to 11.11% （P 〈 0.05 ）. The results were statistically significant. According to the calculation of priority risk coefficient（RPN） , five high and medium risk potential tailure mode OSD scores were reduced to varying degrees, and the risk level was also lowered. Conclusion FMEA method can effectively reduce the risk of nosoeomial infection in the catheter room and prevent the incidence of hospital infection related to interventional surgery.
