摘要
目的探讨全程信息化静脉药物调配中心(PIVAS)运行初期出现的差错情况,加强管理保障静脉输液安全性。方法分析上海市虹口区江湾医院(以下简称"我院")PIVAS 2016年4月~2017年3月运行初期的差错共计511例,汇总分类、找寻原因、提出改进措施,分两阶段检查改进成效。结果我院PIVAS差错主要表现为审方、排药及配置问题;针对不同环节差错实施改进措施后,第二阶段(2016.10~2017.3)较第一阶段(2016.4~2016.9)总差错率环比降低37.28%,不合理医嘱率稳步下降。结论优化PIVAS信息系统、构建自主审方平台、加强人员与制度规范管理等措施。减少差错发生,保障临床用药的安全性、合理性、有效性。
Objective To explore the errors in initial period of informationalized pharmacy intravenous admixture ser- vices （PIVAS） for reinforcing the security of vein transfusion. Methods From April 2016 to March 2017, a total of 511 cases of errors in initial period of informationalized PIVAS in Jiangwan Hospital in Hongkou District （＂our hosipital＂ for short） were analyzed, meanwhile the types of errors, causes, preventive measures were summarized, and the effects were evaluated by two stages. Results The PIVAS errors were mainly occurred in prescription check, drug dispensing, drug admixture. Considering the errors in each step, improvement measures had been implemented. In the second stage （2016.10-2017.3）, the overall error rate decreased by 37.28% compared with that in the first stage （2016.4-2016.9）, and the rate of irrational prescription reduced gradually. Conclusion By optimizing information system, developing in- dependent platform of prescription evaluation, strengthening personnel management and system standordized mauage- ment can reduce the occur of error, and insure the security, rationality, effectiveness of clinical drug use.
