经食管超声引导下胸腔微创封堵婴儿冠状动脉瘘二例 被引量:3

Mini-invasive closure of perventriular coronary artery fistula under the guidance of transesophageal echocardiography: a report of 2 cases

摘要 冠状动脉瘘(coronary artery fistulas,CAF)是一种少见的心脏畸形,是左右冠状动脉主干或分支与心脏四腔或大血管存在异常交通[1]。CAF对血流动力学的影响主要取决于瘘管的位置、形态、直径、瘘管的阻力及其与心腔血管之间的压力阶差等因素,常可在胸骨左源闻及舒张期杂音,出现乏力、心悸、气促、呼吸困难等不同程度的心功能不全的表现。严重情况可引起心肌梗死、冠状动脉瘤形成甚至破裂、猝死以及感染性心内膜炎等并发症[2]。CAF一经发现应尽快手术治疗,目前该病主要治疗手段是心脏直视手术和放射线引导下经皮介入术,两种治疗策略各有利弊[3]。
