摘要
目的评价齿形胶圈套扎术治疗Ⅱ~Ⅲ度混合痔的临床疗效。方法回顾性分析淮安市中医院及金湖县中医院肛肠科2016年1月至2017年11月收治的316例Ⅱ~Ⅲ度混合痔患者的临床资料,按照手术方式的不同分为治疗组(齿形胶圈套扎术)和对照组(M-M),每组各158例,比较两组术后疗效、术后创面并发症情况、舒适度,记录手术时间、手术切口数及创面愈合时间并进行比较。结果治疗组治愈126例、好转32例,对照组治愈115例、好转43例。术后24 h,治疗组舒适度总体优于对照组,组间差异有统计学意义(P<0.05)。术后第1 d、7 d,治疗组疼痛及出血评分低于对照组,术后第7 d、14 d,治疗组水肿评分低于对照组,差异均有统计学意义(均P<0.05)。术后治疗组的手术时间、手术切口数及创面愈合时间均少于对照组,差异均有统计学意义(均P<0.05)。结论与M-M术相比,齿形胶圈套扎治疗Ⅱ~Ⅲ度混合痔可有助于缩短手术时间及创面愈合时间及提升患者的舒适度,并可减轻术后创面的疼痛、出血及水肿程度。
Objective To evaluate the clinical efficacy of dentiform rubber ligation in Ⅱ ~ Ⅲ degree mixed hemorrhoids. Methods The clinical data of 316 patients with Ⅱ~Ⅲ degree mixed hemorrhoids treated in Huaian Hospital of Traditional Chinese Medicine and Jinhu County Hospital of Traditional Chinese Medicine from January 2016 to November 2017 were analyzed retrospectively. According to the surgical methods, patients were divided into treatment group（receiving dentiform rubber ligation） and the control group（receiving M-M）, with 158 cases in each group. The curative effect, the complications of wound, degree of comfort after the operation, operation duration, number of incisions, and time to wound healing were compared between the two groups. Results In the treatment group,126 cases were cured and 32 cases had improved symptoms, whilst 115 cases in the control group were cured and 43 cases had improved symptoms. The overall degree of comfort 24 hours after the operation was significantly better in the treatment group than in the control group（P〈0.05）. Scores of wound pain and bleeding 1 and 7 days after the operation were significantly lower in the treatment group than in the control group（P〈0.05）. Scores of wound edema 7 and 14 days after the operation were significantly lower in the treatment group than in the control group（P〈0.05）. Operation duration, number of incisions, and time to wound healing were significantly shorter or lower in the treatment group than in the control group（P〈0.05）. Conclusion Compared with M-M, dentiform rubber ligation can effectively shorten operation duration and time to wound healing and improve degree of comfort in patients with mixedⅡ~Ⅲ degree hemorrhoids. It can also alleviate postoperative pain, bleeding, and edema of the wound.
