二维斑点追踪显像和实时三维超声心动图评价慢性肾脏病患者左房功能的研究 被引量:6

Evaluation of left atrial function in patients with chronic kidney diseases by two-dimensional speckle tracking imaging and real-time three-dimensional echocardiography

摘要 目的应用二维斑点追踪显像和实时三维超声心动图评价慢性肾脏病(CKD)患者左房功能。方法应用二维斑点追踪显像技术获取47例CKD患者(CKD组)和42例健康体检者(对照组)的左房心肌应变曲线,得出左房总体应变及相应的应变率;同时实时三维超声心动图技术获取左房最大容积(LAVmax)、左房最小容积(LAVmin)及左房收缩期前容积(LAVpre-A),并计算左房总体射血分数(LATEF)、左房被动射血分数(LAPEF)及左房主动射血分数(LAAEF);所得容积参数采用体表面积进行校正,获得相应容积指数(LAVImax、LAVImin、LAVIpre-A)。比较两组上述各指标变化情况。结果与对照组比较,CKD组应变和应变率均明显降低,LAVImax、LAVImin及LAVIpre-A均明显增高,LATEF、LAAEF及LAPEF均明显降低,差异均有统计学意义(均P<0.05)。结论 CKD患者左房功能减退;二维斑点追踪显像和实时三维超声心动图可用于评价CKD患者左房功能变化。 Objective To evaluate the left atrium function in patients with chronic kidney diseases by two dimensionalspeckle tracking imaging(2 D-STI) and real-time three-dimensional echocardiography(RT-3 DE).Methods Forty-seven patients with chronic kidney diseases(CKD group)and 42 healthy individuals(CTL group)were enrolled in this study.The strain and strain rate(Ss,Se,Sa,SRs,Sre,SRa) were measured by 2 D-STI.The maximum volume(LAVmax),the minimum volume(LAVmin),the volume before left atrial contraction(LAVpre-A)were acquired by RT-3 DE,and all volume parameters were corrected by the body surface area.The total(LATEF),passive(LAPEF)and active(LAAEF)ejection fraction of left atrium were calculated.Results Compared with CTL group,the strain and strain rate were significant reduced in CKD group(all P〈0.05).All the volume indexes of CKD group was obviously higher than those of CTL group,but all the ejection fraction were just the reverse(all P〈0.05).Conclusion The left atriumis function reduced in patients with CKD.2 D-STI and RT-3 DE may effectively evaluate the changes of left atrium function.
