摘要
目的 观察盐酸左氧氟沙星滴耳液联合曲安奈德注射液治疗急性化脓性中耳炎的临床疗效.方法 采用随机数字表法将2017年1月至2018年3月医院就诊的68例急性化脓性中耳炎患者分为治疗组与对照组,每组34例,治疗组与对照组均给予口服头孢呋辛酯片基础治疗,对照组采用盐酸左氧氟沙星滴耳液治疗,治疗组在对照组基础上采用鼓室内注射曲安奈德注射液治疗,治疗组与对照组均治疗10 d.结果 治疗组与对照组治疗后C-反应蛋白（CRP）、白细胞介素-8（IL-8）、肿瘤坏死因子-α（TNF-α）均较治疗前降低（P均〈0.05）,且治疗组降低程度大于对照组（P〈0.05）;治疗组耳痛缓解时间、鼓膜充血消退时间及听力恢复时间均短于对照组（P均〈0.05）,治疗组总有效率高于对照组（P〈0.05）,治疗组与对照组不良反应发生率比较差异未见统计学意义（P〉0.05）.结论 盐酸左氧氟沙星滴耳液联合曲安奈德注射液治疗急性化脓性中耳炎能够减轻机体炎症反应程度,改善临床症状,缩短病程,提高临床疗效,且具有较好的安全性.
Objective To observe the clinical efficacy of levofloxacin hydrochloride ear drops combined with triamcinolone acetonide injection on acute suppurative otitis media. Methods Using ran- dom number table method, 68 patients with acute suppurative otitis media in hospital from Jan. 2017 to Mar. 2018 were divided into two groups, 34 cases in each group. Both groups were given oral cefuroxime axetil based therapy. The control group was treated with levofloxacin hydrochloride ear drops. The treatment group was treated with intratympanic injection of triamcinolone acetonide injection on the basis of the control group. Both groups were treated for 10 d. Results After treatment, the levels of CRP, IL-8 and TNF-cx in both groups were lower than those before treatment （ P 〈 0. 05 ）, and the degree of reduc- tion in the treatment group was greater than that in the control group （P 〈 0. 05 ）. The time of earache remission, eardrum hyperemia, and hearing recovery time were shorter in the treatment group than that in the control group （P 〈 0.05 ）. The total effective rate in the treatment group was higher than that in the control group （P 〈 0. 05 ）. The difference in the incidence of adverse reactions between the two groups was not significant （ P 〉 0. 05 ）. Conclusions Levofloxacin hydrochloride ear drops combined with triamcinolone acetonide injection in the treatment of acute suppurative otitis media can reduce the degree of inflammation in the body, improve the clinical symptoms, shorten the course of the disease, improve the clinical efficacy, and has good safety.
