摘要
目的探讨孔源性视网膜脱离患者的裂孔特征及手术方式选择的影响因素。方法回顾性分析我院收治的242例孔源性视网膜脱离入院行手术治疗的患者的临床资料,包括年龄、性别、裂孔大小、数目及象限分布、术前视力、术前黄斑在位、住院费用等因素。分析手术方式选择可能的影响因素及术后视网膜复位情况。结果 242例患者中,以马蹄形裂孔及萎缩孔为主(197/242),术前未发现裂孔8例。131例接受巩膜扣带术治疗,111例行玻璃体切除术治疗。手术方式选择与术前视力、术前黄斑在位、住院费用相关(P<0.05)。巩膜扣带术及玻璃体切除术的一次手术复位率比较,差异无统计学意义(P>0.05)。与玻璃体切除术比较,巩膜扣带术出血发生率更高,白内障加重发生率更低(P<0.05)。结论巩膜扣带术及玻璃体切除术治疗孔源性视网膜脱离均可取得满意效果,但巩膜扣带术更值得临床推广。
Objective To investigate the clinical characteristics of holes and the influencing factors of selection of surgical methods for the patients with rhegmatogenous retinal detachment. Methods The clinical data of 242 patients with rhegmatogenous retinal detachment who treated with surgery were retrospectively analyzed, including age, sex, size, number and quadrant distribution, preoperative visual acuity, preoperative macular presence, cost of hospitalization, et al. The possible factors influencing the selection of surgical methods and the situation of retinal reattachment after operation were analyzed. Results Among 242 patients, the horseshoe hole and atrophy hole were dominant（197/242）, and 8 cases didn＇t find the holes before operation. A total of 131 cases underwent scleral buckling operation, and another 111 cases underwent vitrectomy. The choice of surgical methods was related to preoperative visual acuity, preoperative macular position and hospitalization expenses（P〈0.05）. There was no significant difference in the incidence of postoperative retinal reattachment between patients with scleral buckling operation and patients with vitrectomy（P〈0.05）. Compared with vitrectomy, there were higher incidence of bleeding and lower incidence of cataract aggravation in patients with scleral buckling operation（P〈0.05）. Conclusion Scleral buckling operation and vitrectomy has satisfactory clinical effect in the treatment of rhegmatogenous retinal detachment, but scleral buckling is more worth popularizing.
