摘要
目的：观察活血化瘀中药治疗系统性红斑狼疮（systemic lupus erythematosus,SLE）的临床疗效。方法：将86例SLE患者按随机数字表法分为2组,每组43例,对照组采用糖皮质激素和环磷酰胺治疗,治疗组在对照组的基础上联合活血化瘀中药治疗,连续治疗3个月,比较两组患者治疗前及治疗结束后中医症候积分变化,检测补体C3、C4、T淋巴细胞、B淋巴细胞、NK细胞和ds-DNA抗体的变化。结果：两组中医证候积分下降,治疗组CD3-CD19＋B水平降低,差异有统计学意义（t=7.273,P=0.011;t=8.415,P=0.007;t=5.639,P=0.008）;补体C3、C4水平升高,CD4＋、CD4＋/CD8＋水平升高,差异有统计学意义（t=4.552,P=0.037;t=6.372,P=0.029;t=3.714,P=0.016;t=5.837,P=0.032）;对照组和治疗组抗ds-DNA抗体转阴率分别为30.23%（13例）、44.19%（19例）,差异具有统计学意义（t=6.415,P=0.023;t=5.163,P=0.038）。结论：在西药治疗基础上加用活血化瘀中药治疗系统性红斑狼疮,可改善患者免疫功能,提高临床疗效。
Objective：To observe the clinical efficacy of blood-activating and stasis-removing Chinese medicine in treating systemic lupus erythematosus（SLE）. Methods：Eighty-six cases of SLE patients were divided into two groups according to random number table method,forty-three cases in each group. The control group was treated with glucocorticoid and cyclophosphamide,the treatment group was treated on the basis of control group with blood-activating and stasis-removing Chinese medicine,and the continuous treatment for 3 months. The changes of TCM symptom score and complement C3,C4,T lymphocyte,B lymphocyte,NK cell and ds-DNA antibody before treatment and after treatment were compared between the two groups. Results：The score of two groups of TCM syndromes decreased,and the level of CD3-CD19＋ B in treatment group was reduced,the difference was statistically significant（t=7.273,P=0.011;t=8.415,P=0.007;t=5.639,P=0.008）. Complement C3,C4 and CD4＋,CD4＋/CD8＋ levels increased,the difference was statistically significant（t=4.552,P=0.037;t=6.372,P=0.029;t=3.714,P=0.016;t=5.837,P=0.032）. The anti ds-DNA antibody negative rates in the control group and the treatment group were 30.23%（13 cases）and 44.19%（19 cases）respectively,the difference was statistically significant（t=6.415,P=0.023;t=5.163,P=0.038）. Conclusion：On the basis of Western medicine combined with blood-activating and stasis-removing Chinese medicine treatment of systemic lupus erythematosus can improve immune function and enhance clinical efficacy.
