摘要
目的探讨增强CT扫描甲状腺癌的影像学特点及定性诊断价值。方法回顾性分析该院2014年1月—2017年12月经病理证实的32例甲状腺癌增强CT影像学资料,总结CT扫描的影像学特点,分析CT平扫及增强CT扫描对甲状腺癌病灶的检出率、诊断率,淋巴结转移检出率等差异,分析增强CT扫描对甲状腺的定性诊断价值。结果甲状腺癌在CT平扫主要表现为不规则实性肿物、密度不均匀(84.4%),结节外浸征象(59.4%),钙化灶(53.1%),在增强CT扫描主要表现为增强后病灶范围缩小(81.2%),明显强化(90.6%),钙化灶中断(74.0%),32例甲状腺癌病灶34处,CT平扫发现病灶29处(85.3%),定性诊断14例(43.8%),增强CT扫描发现病灶34处(100.0%),定性诊断30例(93.8%),增强CT扫描病灶检出率高于平扫CT(χ~2=5.397,P=0.020),定性诊断率均高于平扫CT(χ~2=18.618,P=0.000),CT平扫淋巴结检出率为(73.7%),CT增强扫描淋巴结检出率为(77.2%),CT平扫同CT增强扫描淋巴结检出率差异无统计学意义(χ~2=0.189,P=0.663)。结论增强CT扫描甲状腺癌具有特征性的影像学特点,能够为进行较好的定性诊断。
Objective This paper tries to investigate the imaging features and qualitative diagnostic value of enhanced CT scan for thyroid cancer. Methods The CT imaging data of 32 cases of thyroid cancer confirmed by pathology from January 2014 to December 2017 in the hospital were retrospectively analyzed, summarized the imaging features of CT scan, and analyzed the detection of thyroid cancer lesions by CT plain scan and enhanced CT scan. The check rate, di- agnostic rate, lymph node metastasis detection rate and other differences, analyzed the enhanced CT scan thyroid quali- tative diagnostic value. Results Thyroid carcinoma on CT plain scan mainly showed irregular solid tumors, uneven den- sity (84.4%), extranodal immersion signs (59.4%), calcification (53.1%), mainly on enhanced CT scans. The lesions were narrowed (81.2%), significantly enhanced (90.6%), calcified lesions were interrupted (74.0%), 32 cases of thyroid cancer lesions, 29 lesions (85.3%) were detected by CT scan, and qualitative diagnosis was 14 cases (43.8%). Enhanced CT scan found 34 lesions (100.0%), qualitative diagnosis of 30 cases (93.8%), enhanced CT scan lesion detection rate was higher than plain CT (X2=5.397, P=0.020), qualitative diagnosis rate was higher than plain CT (X2=18.618,P=0.000), CT scan lymph node detection rate (73.7%), CT scan lymph node detection rate (77.2%), CT plain scan with CT scan lymph node detection rate was no significant difference (X2=0.189, P=0.663). Conclusion Enhanced CT scans of thyroid cancer have characteristic imaging features that enable better qualitative diagnosis.
