摘要 目的探讨免疫组化蛋白标志物在甲状腺乳头状癌(PTC)组织中的表达及其在鉴别诊断和预后评估中的价值。方法利用免疫组化方法检测130例PTC和103例甲状腺良性病变(BTL)组织中15种临床常用的免疫组化蛋白标志物。结果 CK19、CK34βE12、MC、galectin-3、CA19-9、CD57、Cyclin D1在PTC组织中的表达明显高于BTL(P均<0.05);CD56、TPO、p27kip1在PTC组织中的表达明显低于BTL(P均<0.05);CK7、CK8/18、EMA、Tg、TTF-1在PTC及BTL组织中均高表达(P均>0.05)。PTC组织中p27kip1和Cyclin D1表达与肿瘤淋巴结转移与否及肿瘤分期有关(P均<0.05)。结论 CK19、CK34βE12、MC、galectin-3、CA19-9、CD56、CD57、TPO、p27kip1和Cyclin D1对PTC的鉴别诊断具有重要临床意义,其中p27kip1丢失和Cyclin D1表达上调可作为评估PTC预后的参考指标。 Objective To investigate the value of immunohistochemical markers in the differential diagnosis and prognosis evaluation of papilary thyroid carcinoma( PTC). Methods The 15 kinds of protein expression were detected by immunohistochemical method in the 130 cases of PTC and 103 cases of benign thyroid lesions( BTL). Results The expression rates of CK19,CK34βE12,MC,galectin-3,CA19-9,CD57,Cyclin D1 protein in the PTC were significantly higher than those in the benign thyroid lesions( P〈0.05). The expression rates of CD56,TPO,p27^kip1 in the PTC were lower than those in the BTL( P〈0.05). CK7,CK8/18,EMA,Tg and TTF-1 were all highly expressed in the PTC and the BTL,there was no statistical differences( P〉0.05). The p27^kip1 and Cyclin D1 expression were related with lymphatic metastasis and TNM stage( P〈0.05). Conclusion CK19,CK34βE12,MC,galectin-3,CA19-9,CD56,CD57,TPO,p27^kip1 and Cyclin D1 have important clinical significance in the differential diagnosis of PTC. The negative p27 kip1 and high Cyclin D1 expression could be used as a reference indicator for prognosis evaluation of PTC.
