摘要
目的:观察显微镜辅助下颈前路椎间盘切除植骨融合术治疗单节段颈椎病的临床疗效。方法:选取我院于2016年2月至2017年7月期间收治单节段颈椎病患者98例。按照随机数字表法分为对照组和研究组,分别为48例、50例。对照组采用标准颈前路椎间盘切除植骨融合术治疗,研究组采用显微镜辅助下颈前路椎间盘切除植骨融合术治疗。比较两组手术相关指标和治疗前后视觉模拟评分(Visual analogue scale,VAS)、日本矫形外科协会评分(Japanese orthopaedics association,JOA)及并发症发生情况。结果:研究组术中出血量少于对照组,差异具有统计学意义(P<0.05);与治疗前比较,治疗后研究组和对照组VAS评分明显下降,而JOA评分明显上升,且研究组上述观察指标均优于对照组,差异具有统计学意义(P<0.05);研究组并发症发生率为2.00%(1/50),与对照组8.33%(4/48)相比,差异无统计学意义(P>0.05)。结论:对单节段颈椎病患者采用显微镜辅助下颈前路椎间盘切除植骨融合术治疗,可显著提高临床疗效,减少术中出血量,且不会增加并发症,减轻患者痛苦,促进颈椎功能的恢复。
Objective： To observe the clinical efficacy of microscope-assisted anterior cervical discectomy and fusion（ACDF） in treatment of single segmental cervical spondylosis. Methods： 98 cases of single segment cervical spondylosis admitted to our hospital from February 2016 to July 2017 were selected. According to the random number table,they were divided into the control group and the research group,48 cases and50 cases respectively. Control group was treated with standard ACDF,research group was treated with microscope-assisted ACDF. The surgical indexes,changes of visual analogue scale（VAS）,Japanese orthopaedics association（JOA） and complications of two groups before and after treatment were compared. Results： The intraoperative bleeding volume of research group was less than that of control group（P 0.05）; Compared with before treatment,the VAS scores of two groups significantly decreased after treatment,but the JOA score significantly increased,and above indexes of research group were better than those of control group（P 0.05）;The incidence of complications of research group was 2.00%（1/50）,which compared with 8.33%（4/48） of control group,there was no statistical difference（P 0. 05）. Conclusion： The treatment of single cervical spondylosis with microscope assisted anterior cervical discectomy and fusion can significantly improve the clinical effect,reduce the amount of bleeding in the operation,and will not increase the complications,relieve the pain of the patients and promote the recovery of the function of the cervical vertebra.
