摘要
目的通过分析老年2型糖尿病患者服药依从性的影响因素并实施MTM模式干预,探索符合社区老年2型糖尿病患者新的管理模式。方法选取老年2型糖尿患者55例,分析服药依从性的危险因素,于2015年1月~2016年1月期间实施MTM模式管理,分析干预前、干预3个月及干预结束时各项指标变化情况。结果通过MTM模式干预,患者在服药依从率、糖尿病知识知晓率、自我管理水平方面均较干预前提高(P<0.05);在抑郁状况以及血糖、血压、体质指数达标率方面,干预结束时各指数均较干预前降低(P<0.05);干预结束时服药时间偏差率、数量偏差率、漏服药率、服医嘱外药率均明显低于干预前(P<0.05)。结论实施MTM模式可有效提高患者服药依从性,改善药物治疗效果,可作为老年2型糖尿病患者的日常管理模式。
Objective By analyzing the influencing factors of medication compliance in elderly patients with type 2 diabetes mellitus and implementing MTM model intervention, to explore the new management model of elderly patients with type 2 diabetes mellitus in community.Methods Fifty-five patients with type 2 diabetes mellitus were enrolled in this study, the risk factors for medication adherence were analyzed. MTM model intervention was administered from January 2015 to January 2016, and the changes of indicators before intervention, 3 months after intervention and the end of intervention were analyzed. Results Through MTM model intervention, the patients＇ compliance rate, awareness rate of diabetes knowledge and self-management level were higher than those before intervention（P〈0.05）. In terms of depression, blood glucose, blood pressure and body mass index, the indexes at the end of intervention were lower than those before intervention（P〈0.05）. At the end of the intervention, the time deviation rate, the quantity deviation rate, the rate of missing medication and the rate of taking out-of-order medication were significantly lower than those before the intervention（P〈0.05）. Conclusion The implementation of MTM model can effectively improve the patient medication compliance, improve the effect of drug treatment, can be used as a daily management mode for elderly patients with type 2 diabetes mellitus.
