摘要
目的探讨血清肿瘤标志物在预测胃癌病理学分期的应用价值。方法分析2014年8月至2017年8月广东省东莞东华医院收治的174例胃癌患者。所有患者均经病理组织活检确诊并分期,均检测血清肿瘤标志物CEA、CA125、CA72-4、CA19-9水平。分析CEA、CA125、CA72-4、CA19-9肿瘤标志物在不同病理学分期胃癌患者中的表达以及阳性表达率,分析其与胃癌病理分期的相关性。结果血清肿瘤标志物CEA、CA125、CA72-4、CA19-9在Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期胃癌患者中的表达水平依次增高,差异有统计学意义(P<0.05)。CEA、CA125、CA72-4、CA19-9阳性表达率在Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期胃癌患者中依次增高,CEA、CA125、CA72-4、CA19-9阳性表达率与病理分期呈正相关,r值分别为0.691、0.582、0.768、0.550,P值分别为0.000、0.004、0.000、0.006,差异有统计学意义(P<0.05)。结论血清肿瘤标志物CEA、CA125、CA72-4、CA19-9在预测胃癌病理学分期中具有较好的应用效果,对其进行监测,能为其诊断及病情评估提供参考。
Objective To study the value of serum tumor markers in predicting the pathological stage of gastric cancer. Methods A total of 174 patients with gastric cancer treated in our hospital from August 2014 to August 2017 were retrospectively analyzed. The pathological tissue biopsies of all patients were diagnosed and staged,and the levels of serum tumor markers CEA,CA125,CA72-4 and CA19-9 were detected. The specific expression and positive expression rate of tumor markers of CEA,CA125,CA72-4 and CA19-9 from the patients with gastric cancer in different pathological stages were analyzed and the correlation between the pathological stage of gastric cancer and the pathological stage of gastric cancer was analyzed. Results The expression level of serum tumor markers CEA,CA125,CA72-4 and CA19-9 from the patients with stage Ⅰ,Ⅱ,Ⅲ andⅣ gastric cancer increased sequentially,and the difference was statistically significant（ P〈0. 05）. The positive expression rate of CEA,CA125,CA72-4 and CA19-9 in patients with gastric cancer was positively correlated with the pathological stage of gastric cancer（ r = 0. 691,0. 582,0. 768,and 0. 550,respectively）,and the difference was statistically significant（ P = 0. 000,0. 004,0. 000,and 0. 006,respectively）. Conclusions Serum tumor markers CEA,CA125,CA72-4 and CA19-9 have better application effect in predicting the pathological stage of gastric cancer. Monitoring of them can provide references for their diagnosis and disease evaluation.
