摘要
目的探讨使用补肺化痰健脾膏联合沙美特罗替卡松对慢性阻塞性肺病(COPD)稳定期(中医辨证为脾肺气虚型)的临床疗效。方法将95例COPD患者按照随机数字表法分成2组,对照组采用常规药物治疗,治疗组采用常规药物加化痰补肺健脾膏治疗,疗程均为3个月。观察并比较2组患者治疗前和治疗1、2、3个月时慢性阻塞性肺病评估测试(CAT)评分和肺功能指标。结果治疗1、2个月时,2组患者CAT评分、第一秒用力肺活量(FEV1值)、FEV1占预计值百分比与治疗前比较差异均无统计学意义(P>0.05);治疗3个月时,治疗组患者CAT评分均明显低于治疗前和同期治疗的对照组(P<0.05),治疗组患者FEV1值、FEV1占预计值百分比均比对照组有明显上升(P<0.05),也高于治疗前(P<0.05)。结论中医辨证为脾肺气虚型,使用自制中药膏方"化痰补肺健脾膏"结合沙美特罗替卡松对慢性阻塞性肺病(COPD)稳定期能有效改善患者肺功能。联合疗法的疗程不应低于3个月。
Objective To investigate the clinical efficacy of Bufei Huatan Jianpi ointment combined with salmeterol-fluticasone in treating stable stage chronic obstructive pulmonary disease（ COPD）,which is belonged to syndrome differentiation of TCM,spleen and lung qi deficiency. Method 95 patients with COPD were divided into 2 groups according to the random number table. The control group was treated with conventional medicine. The treatment group was treated with conventional medicine combined with Bufei Huatan Jianpi ointment.The course of treatment was 3 months. Chronic obstructive pulmonary assessment test（ CAT） score and lung function index were observed and compared between the two groups before and after treatment at 1 st,2 nd and 3 rd month. Result At 1 st and 2 nd month after treatment,the CAT score,the first forced vital capacity（ FEV1） value and the percentage of FEV1 in the 2 groups were not significantly different from those before treatment（ P〉0. 05）. For 3 rd month,the patients in the treatment group CAT scores were significantly lower than the control group before treatment and treatment over the same period（ P〈0. 05）,the patients in the treatment group FEV1,FEV1 percentage of predicted value was higher than that of the control group,which had significantly increased（ P〈0. 05）,also higher than before treatment（ P〈0. 05）. Conclusion The combination of Bufei Huatan Jianpi ointment combined with salmeterol-fluticasone can effectively improve COPD patients with stable pulmonary function,and the duration of combination therapy should not be less than three months.
