摘要
目的观察骨化三醇配伍阿仑膦酸钠治疗绝经后骨质疏松症患者对骨密度、骨代谢及基质金属蛋白酶-13(MMP-13)、组织金属蛋白酶抑制因子-1(TIMP-1)的影响。方法将2015年1月—2016年12月湖北省荆门市第二人民医院妇科收治的绝经后骨质疏松症患者60例,随机信封法分为观察组及对照组各30例。对照组采用阿仑膦酸钠治疗,观察组采用骨化三醇配伍阿仑膦酸钠治疗,疗程6个月。观察并比较2组治疗前后骨痛程度(VAS评分),股骨颈、腰椎正位、股骨粗隆的骨密度水平,骨型碱性磷酸酶(BALP)、酸性磷酸酶异构体5b(TRACP-5b)、MMP-13及TIMP-1水平。结果观察组患者治疗后VAS评分显著低于对照组(t=9.487,P=0.000);治疗后观察组腰椎正位、股骨颈及股骨粗隆处骨密度均明显高于对照组,差异有统计学意义(t=14.033、3.988、5.317,P均=0.000);治疗后,观察组血中BALP及TRACP-5b水平显著低于对照组(t=2.323、4.400,P均<0.05);治疗后观察组血中MMP-13及MMP-13/TIMP-1水平显著低于对照组(t=19.321、2.903,P<0.01),TIMP-1水平显著高于对照组(t=2.113,P=0.036)结论绝经后骨质疏松患者采用骨化三醇联合阿仑膦酸钠治疗后,可有效提高临床疗效,并调控MMP-13、MMP-13/TIMP-1水平。
Objective To observe the effect of ossification of three alcohol combined with alendronate on bone metabolism and MMP-13 and MMP-13/TIMP-1 in postmenopausal women with osteoporosis, and to explore the mechanism of treatment,so as to provide evidence for clinical application. Methods From January 2015 to December 2016, 60 eases of osteoporosis who were admitted to the department of Gynecology of Jingmen Second People's Hospital of Hubei province were randomly divided into observation group and control group; the control group using alendronate treatment,observation group with ossification in three alcohol combined with alendronate sodium. Before and after treatment, the VAS method is applied to evaluate the degree of pain, detect of femoral neck, lumbar spine, femoral trochanter bone density, bone alkaline phosphatase(BALP), TRACP 5 and MMP-13 and TIMP-1 level. Results After treatment, the VAS scores in observation group were significantly lower than the control group(t=9.487,P =0.000); observation group after treatment, lumbar spine, femoral neck and trochanter bone density were significantly higher than the control group, and the difference is statistical significance(t=14.033,P = 0.000; t=3.988,P=0.000; t = 12.660,P =0.000); after treatment, observation group's BALP and the level of TRACP 5 b in the blood was significantly lower than the control group(t = 2. 323, P =0. 024; t =4. 400, P =0. 000);MMP-13 and MMP-13/TIMP-1 levels after treatment in the observation group was significantly lower than that in the control group(t = 19.321, P = 0.000; t=2903, P=0.006). After treatment, the level of TIMP-1 in the observation group was significantly higher than that in the control group(t=2. 113, P= 0.036). Conclusion In postmenopausal osteoporosis, the treatment of three alcohol combined with alendronate can effectively improve the clinical efficacy, and regulate the levels of MMP-13 and MMP-13/TIMP-1.
