摘要
目的探讨米索前列醇用于预防经阴道自然分娩产后出血的效果。方法从该院2016年12月—2017年11月产科收治的产妇中选取68例进行统计学数据研究分析,随机分为两组,两组产妇经阴道自然分娩后,给予对照组产妇注射15 U宫缩素,观察组产妇给予口服米索前列醇片,观察两组产妇产时、产后2 h、产后24 h出血量和不良反应(头痛、发热、腹泻、呕吐)发生率。结果观察组产妇产时、产后2 h、产后24 h出血量分别为(182.1±20.1)、(104.2±22.1)、(282.1±48.3)mL,对照组产妇产时、产后2 h、产后24 h出血量分别为(256.1±31.2)、(201.1±34.1)、(476.3±46.1)mL,以上数据观察组均显著低于对照组产妇,相比差异有统计学意义(t=7.49、8.41、6.37)(P<0.05。观察组产妇不良反应发生率仅为5.9%,对照组产妇不良反应发生率高达29.4%,相比差异有统计学意义(χ~2=11.29,P<0.05)。结论在经阴道自然分娩产后出血中,采用米索前列醇进行干预,可以显著降低产妇的产后出血量,并且能降低产妇的不良反应发生率,安全性较强,值得临床推广。
Objective To investigate the effect of misoprostol on prevention of postpartum hemorrhage after vaginal delivery. Methods 68 cases of delivery women from December 2016 to November 2017 in our hospital were selected for statistical research, and were randomly divided into two groups, and the control group received 15 U injection of oxytocin, while the observation group took misoprostol tablets orally, and the bleeding amount at 2 h and 24 h at delivery and after delivery and incidence rate of adverse reactions such as headache, fever, diarrhea and vomiting of the two groups were observed. Results The bleeding amount of delivery women in the observation group at delivery, in 2 h and24 h after delivery were obviously lower than those in the control group, [（182.1±20.1）,（104.2±22.1）,（282.1±48.3）mL vs（256.1±31.2）,（201.1±34.1）,（476.3±46.1）mL], and the differences were obvious（t=7.49, 8.41, 6.37,P〈 0.05）, and the incidence rate of adverse reactions in the observation group was obviously lower than that in the control group,（5.9% vs29.4%）（χ2=11.29, P〈 0.05）. Conclusion The intervention of misoprostol in postpartum hemorrhage after vaginal delivery can significantly reduce the amount of postpartum hemorrhage and reduce the incidence of maternal adverse reactions,which is safe and worthy of clinical promotion.
