摘要
目的探讨腹腔镜下卵巢肿瘤剥除术中使用静脉全麻和腹横肌阻滞复合全麻对患者免疫功能的影响。方法选择2017年6—12月间在该院择期治疗并实行腹腔镜下卵巢肿瘤剥除术后的100例患者,使用随机的方式分为观察组和对照组,每组各50例,其中对对照组患者使用静脉全麻的麻醉方式,对观察组患者使用腹横肌阻滞复合全麻的麻醉方式,对比分析患者的免疫功能受不同麻醉方式的影响程度。结果首先,两组患者在麻醉前体内CD3,CD4,CD8差异无统计学意义(P>0.05)。两组患者在术后1、3、5、7 d后体内的CD3,CD4,CD8含量均显著下降,其中观察组5 d后体内CD3含量为(44.52±8.5),对照组5 d后体内CD3含量为(39.52±7.6)(t=3.245;P=0.001)。且两组患者在术后的体内上述指标差异较为显著,观察组的指标优于对照组,差异有统计学意义(P<0.05)。结论为实行腹腔镜下卵巢肿瘤剥除术的患者实行腹横肌阻滞符合全麻的麻醉方式效果显著,能够减少麻醉药物的用量,减少其对呼吸,循环和中枢神经系统功能的影响,能够降低免疫功能中受到的抑制作用,适用于临床的广泛推广。
Objective To study the effect of intravenous general anesthesia and transversus abdominis block in the laparoscopic ovarian neoplasms enucleation on the immune function of patients. Methods 100 cases of patients after the laparoscopic ovarian neoplasms enucleation treated in our hospital from June to December 2017 were selected and randomly divided into two groups with 50 cases in each, the control group used the general anesthesia, while the observation group were treated with transversus abdominis block, and the effects of different anesthesia ways on the immune function were compared and analyzed. Results There were no statistically significant differences in the CD3,CD4,CD8 between the two groups before anesthesia(P〉0.05), and the constants of CD3,CD4,CD8 in 1,3,5,7 d after surgery obviously decreased, and the CD3 constant in 5 d in the observation group and in the control group was respectively(44.52±8.5) and(39.52±7.6)(t=3.245;P=0.001), and the differences in the above indicators after surgery between the two groups were obvious, and the indicators in the observation group were better than those in the control group, with statistically significant(P 〈0.05). Conclusion The effect of transversus abdominis block in the laparoscopic ovarian neoplasms enucleation patients is obvious, which can reduce the dosage of anesthesia drugs, reduce the effect on the respiration, circulation and central nerve function, and reduce the suppression effect in the immune function, and it is suitable for the wide promotion in clinic.
