摘要
目的:研究入住重症监护室(ICU)心衰患者的中医证型分布与中心静脉压(CVP)和血浆N末端原脑利钠肽(NTpro BNP)的关系,探讨CVP监测对ICU心衰患者中医证型和病情的提示价值。方法:将ICU心衰患者进行中医辨证分型,测量并记录其CVP和血浆NT-pro BNP,同一患者可进行多次测量和辨证,对主要证型组的检测指标进行分析。结果:ICU心衰患者以心肺气虚、痰饮阻肺、阴竭阳脱3种证型为主,各证型的CVP值和NT-pro BNP总体有显著性差异,心衰证型与CVP和NT-pro BNP呈正相关,CVP值随心肺气虚、痰饮阻肺、阴竭阳脱的顺序依次升高。结论:在心肺气虚、痰饮阻肺和阴竭阳脱3种证型的ICU心衰患者中,CVP对3种证型有提示作用,结合NT-pro BNP分析CVP对病情的严重程度和预后亦有提示作用。
Objective： To investigate the relationship of Traditional Chinese Medicine（TCM） syndrome classification with central venous pressure（CVP） and N-terminal prohormone of brain natriuretic peptide（NT-pro BNP） in patients with heart failure in ICU,exploring the value of CVP in TCM syndrome types and disease of ICU patients with heart failure.Method： The patients with heart failure were TCM syndrome differentiated,and the data of every patient on CVP and NTpro BNP were recorded. The same patient can be multiple measured and differentiated and main certificate type groups were analyzed. Results： The 3 main TCM type groups of ICU heart failure patients were Heart-Lung Qi deficiency,phlegm blocking in lung and Yin-Yang exhausted simultaneously. CVP and NT-pro BNP of the 3 syndromes have significant difference. The 3 heart failure syndrome types and CVP were positively correlated,the 3 syndrome types and NT-pro BNP also positively correlated. CVP increased with the syndrome patterns called Heart-Lung Qi deficiency,phlegm blocking in lung and yin-yang exhausted simultaneously in the order. Conclusion： CVP can implicate TCM syndrome types,severity and prognosis of patients suffered from heart failure in ICU among the three（Heart-Lung Qi deficiency,phlegm blocking in lung and Yin-Yang exhausted simultaneously） syndrome types combined with NT-pro BNP.
