超声引导甲状腺肿物细针穿刺细胞学和术中快速冰冻切片组织学检查的对比研究 被引量:8

摘要 目的:探讨超声引导甲状腺肿物细针穿刺细胞学和术中快速冰冻切片组织学检查的对比情况。方法:回顾性分析2016年1月-2018年1月本院收治的甲状腺肿物患者60例的临床资料,观察甲状腺肿物细针穿刺细胞学、术中快速冰冻切片组织学检查和术后常规组织病理学的诊断结果情况。结果:甲状腺肿物细胞学诊断良性病变46例,恶性肿瘤9例;术中快速冰冻切片组织学诊断良性病变48例,恶性肿瘤7例;术后常规组织病理学诊断良性病变47例,恶性肿瘤13例。甲状腺肿物细针穿刺细胞学检查准确性、敏感性、特异性分别为93.3%、84.6%、95.7%,术中快速冰冻切片组织学检查准确性、敏感性、特异性分别为91.7%、69.2%、97.9%,但两种检查准确性、敏感性、特异性比较差异均无统计学意义(P>0.05)。结论:超声引导下甲状腺肿物细针穿刺能准确定位甲状腺肿物,取得甲状腺肿物细胞送病理检查,且其操作方便,安全,微创,诊断准确率高,患者容易接受,可以和术中快速冰冻切片组织学检查一起对诊断结果相互印证,相互补充,具有重要的临床应用价值。 Objective:To study the comparison of ultrasound-guided fine needle aspiration cytology of thyroid neoplasms and histopathological examination of intraoperative rapid frozen section.Method:The clinical data of 60 patients with thyroid neoplasms treated in our hospital from January 2016 to January 2018 were analyzed retrospectively.The diagnostic results of fine needle aspiration cytology,rapid frozen section histological examination and postoperative histopathological were observed.Result:Thyroid tumor cytology diagnosis of benign lesions in 46 cases,malignant tumors in 9 cases.Intraoperative rapid frozen section histological diagnosis of benign lesions in 48 cases,malignant tumors in 7 cases.Postoperative conventional histopathology diagnosis of benign lesions in 47 cases,malignant tumors in 13 cases.The accuracy,sensitivity and specificity of fine needle aspiration cytology were 93.3%,84.6% and 95.7% respectively.The accuracy,sensitivity and specificity of intraoperative rapid frozen section biopsy were 91.7%,69.2%,97.9% respectively.But the accuracy,sensitivity and specificity of two tests were compared,the difference was not statistically significant(P〉0.05).Conclusion:Ultrasound guided fine needle aspiration of thyroid neoplasms can accurately locate the thyroid tumor and obtain the thyroid tumor cells for pathological examination,and the operation is convenient,safe,minimally invasive,high diagnostic accuracy,and can be easily accepted by patients,can be combined with the rapid frozen section histological examination to verify the results and complement each other. It has important clinical value.
