强化护理干预对维持性血液透析高钾血症患者血钾水平的影响 被引量:10

Effect of Intensive Nursing Intervention on Blood Potassium Level in Patients with Maintenance Hemodialysis Hyperkalemia

摘要 目的:探讨强化护理干预对维持性血液透析(MHD)高钾血症患者血钾水平的影响。方法:选取2015年3月至2017年1月收治的MHD高钾血症患者48例,分为对照组与干预组各24例。对照组实施常规透析护理,干预组在此基础上实施强化护理干预。比较2组实施护理干预前后血钾水平变化。结果:干预组的血钾水平低于对照组(P<0.05),随着时间的延长,血钾水平呈下降趋势(P<0.05)。血钾水平在分组与时间上存在交互效应(P<0.05),干预组血钾水平的降低比对照组更明显(P<0.05)。结论:运用强化护理干预对MHD患者高钾血症发生原因进行分析评估,有针对性地制定并执行措施,能有效地降低透析患者血钾水平。 [Objective]To investigate the effect of enhanced nursing intervention on blood potassium level in patients with maintenance hemodialysis(MHD)hyperkalemia.[Method]There were 48 MHD patients complicated with hyperkalemia enrolled in this study from Oct 2015 to Jan2017.They were categorized into control group and intervention group.The patients in the control group were treated with the 6-month regular nursing intervention to prevent hyperkalemia.The patients in the intervention group were given6-month intensive nursing intervention to decrease potassium.The changes of blood potassium levels before and after the implementation of nursing intervention in the two groups was compared.[Result]The serum potassium level in the intervention group was lower than that in the control group with statistically significant difference(P=0.004),and the level decreased with time(P〈0.05).There was an interaction between blood potassium level and group time(P〈0.05).The decrease of serum potassium levels in the intervention group was more significantly than that in the control group(P〈0.05).[Conclusion]Intensive nursing intervention is useful to find and evaluate the causes of hyperkalemia in the patients with maintenance hemodialysis.The corresponding measures can then decrease the serum potassium level effectively,and reduce the risk of sudden cardiac death related with hyperkalemia.
