摘要
目的分析孕妇血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、血管内皮细胞粘附分子-1(CVAM-1)在胎膜早破中的临床意义。方法回顾性分析我院自2016年3月至2017年3月收治93例胎膜早破孕妇临床资料,作为观察组;另选同期93例正常孕妇作为对照组。采取酶联免疫吸附法测定血清CRP、TNF-α、CVAM-1,比较两组孕妇不同破膜时间及有无并发绒毛膜羊膜炎的胎膜早破孕妇的血清CRP、TNF-α、CVAM-1水平;采用Spearman相关性分析胎膜早破孕妇血清CRP、TNF-α与CVAM-1表达的相关性。结果两组年龄、孕周、产程、孕次、新生儿体重差异均无统计学意义(P>0.05);观察组血清CRP、TNF-α、CVAM-1水平均高于对照组(P<0.05);观察组不同破膜时间孕妇血清CRP、TNF-α、CVAM-1水平随着破膜时间而提高,破膜时间<12h、12~24 h、>24h的孕妇血清CRP、TNF-α、CVAM-1水平存在差异(P<0.05);绒毛膜羊膜炎组血清CRP、TNF-α、CVAM-1水平均高于非绒毛膜羊膜炎组(P<0.05);Spearman相关分析,胎膜早破孕妇血清CRP与TNF-α、TNF-α与CVAM-1、CRP与CVAM-1表达均呈正相关(P<0.05)。结论孕妇血清CRP、TNF-α、CVAM-1水平升高与胎膜早破密切相关,三者在胎膜早破发病中可能起到协同作用,及时检测其血清水平有利于评估胎膜早破预后,指导治疗。
Objective To analyze the clinical significance of serum C-reactive protein (C-reactive protein, CRP), tumor necrosis factor-α (Tumor necrosis factor-α, TNF-α)and vascular endothelial cell adhesion molecule-1 (Cell vascular adhesion molecule-1, CVAM-1 ). Methods The clinical data of 93 pregnant women with placental premature rupture who were treated in our hospital from March 2016 to March 2017 were retrospectively analyzed. 93 pregnant women in the same period were selected as the control group. The levels of serum CRP, TNF-α and CVAM-1 in serum of pregnant women were detected with enzyme-linked immunosorbent assay, and compared between the observation group and the control group, with different rupture time and with or without chorioamnionitis. The correlation between placental premature rupture of pregnant women and serum levels of CRP, TNF-α and CVAM-1 were analyzed by Spearman correlation analysis of. Results There was no significant difference in body weight between the observation group and the control group ( P 〉 0.05 ). The levels of CRP, TNF- α and CVAM- 1 in the observation group were higher than those in the control group ( P 〈 0.05 ). The levels of serum CRP, TNF- v and CVAM- 1 in the pregnant women with different rupture time of the observation group increased following the time of rupture, Serum levels of CRP,TNF-α and CVAM-1 in pregnant women with different rupture time ( 〈 12h, 12 -24h, 〉 24h) were significantly different ( P 〈 0. 05 ). The levels of serum CRP, TNF-α and CVAM-1 in chorioamnionitis group were all significantly higher tha those in non-chorioamnionitis group, and the difference was statistically significant ( P 〈 0.05 ). The Spearman correlation analysis showed that the serum levels of CRP, TNF- α, TNF- αand CVAM- 1, CRP and CVAM- 1 expression were positively correlated, both with statistical significance ( P 〈 0.05). Conclusion Elevated serum levels of CRP, TNF- α and CVAM- 1 in pregnant women are closely related to premature placenta previa. They may play synergistic roles in the pathogenesis of placental premature rupture of the placenta. Detecting their serum levels in time is helpful to evaluate the prognosis of placental premature rupture and guide treatment.
