摘要
目的:探讨血清糖类抗原(CA)153、癌胚抗原(CEA)和细针吸取细胞学(FNAC)联合检测在乳腺癌诊断中的价值。方法:选取67例乳腺癌病人和32例乳腺良性疾病病人,分别做血清CA153、CEA和FNAC检测,分析3项指标联合检测对乳腺癌的诊断价值。结果:CA153检测的敏感性为8.96%,特异性为100.00%,准确性为38.38%;CEA敏感性16.42%,特异性96.88%,准确性为43.43%;FNAC的敏感性为83.58%,特异性为100.00%,准确性为88.89%;3项联检敏感性提高到88.06%,准确性提高到90.91%,特异性也达到了96.88%。FNAC、CA153、CEA单项检测诊断乳腺癌的曲线下面积(AUC)分别为0.910、0.709和0.705,3项联合诊断的AUC(0.942)高于单项指标AUC。结论:CA153、CEA和FNAC联合检测可以提高乳腺癌诊断的灵敏度和准确性,对乳腺癌的早期诊断有着较高的参考价值。
Objective: To investigate the value of the detection of serum carbohydrate antigen( CA) 153 and carcinoembryonic antigen( CEA) combined with fine needle aspiration cytology( FNAC) in the diagnosis of breast cancer. Methods: The serum levels of CA153,CEA,and FNAC in 67 patients with breast cancer and 32 patients with benign breast disease were detected. The values of the combined detection of CA 153,CEA and FNAC in the diagnosis of breast cancer were analyzed. Results: The sensitivity,specificity and accuracy of CA153 examination were 8. 96%,100. 00% and 38. 38%,respectively. The sensitivity,specificity and accuracy of CEA examination was 16. 42%,96. 88% and 43. 43%,respectively. The sensitivity,specificity and accuracy of FNAC examination were 83. 58%,100. 00% and 88. 89%,respectively. The sensitivity,specificity and accuracy of three combined inspection were 88. 06%,90. 91% and96. 88%,respectively. The AUC values of CA 153,CEA and FNAC in the diagnosis of breast cancer were 0. 910,0. 709 and 0. 705,respectively,and the AUC value of combined detection( 0. 942) was significantly higher than that of any signal marker. Conclusions:The combined detection of CA153,CEA and FNAC can improve the sensitivity and accuracy in the diagnosis of breast cancer,which has a high reference value in the early diagnosis of breast cancer.
