加温CO_2气体对腹腔镜手术新生儿及小婴儿体温的影响 被引量:27

Effects of warmed carbon dioxide insufflations in laparoscopic surgery for neonates and infants on body temperature

摘要 目的本研究观察注入加温CO_2气体对接受腹腔镜手术的新生儿及小婴儿体温的影响。方法选取天津市某儿童医院2016年1月—2017年5月新生儿外科腹腔镜手术患儿56例作为研究对象,将注入加温后CO_2气体的32例设为实验组;将注入室温CO_2气体的24例设为对照组。观察两组的机体核心温度、术中失血量、血氧饱和度、心率及术后恢复情况。结果两组术前体温(即入手术室体温)和建立CO_2气腹前体温、手术时长,术后体温不升、心率增快(大于180次/min)、血氧饱和度<90%及切口感染的例数差异均无统计学意义(P>0.05)。关闭气腹时与术前相比,实验组体温下降(0.70±0.28)℃,对照组体温下降(1.24±0.23)℃,对照组体温下降幅度明显高于实验组,差异具有统计学意义(t=7.695,P<0.001);对照组术中失血量较实验组多,术后肠道功能恢复时间及住院时间均较实验组长,差异均具有统计学意义(P<0.05)。结论在新生儿和小婴儿腹腔镜手术中注入加温CO_2气体,有利于维持患儿术中体温恒定,避免患儿术后发生低体温,有利于减少术中失血量,促进术后肠道功能恢复,缩短住院时间。 Objective To investigate the effects of warmed carbon dioxide (CO2) insufflations in laparoscopic surgery for neonates and infants on body temperature. Methods From January 2016 to May 2017,56 newborns undergoing laparoscopic surgery were enrolled in our study. The observation group with 32 cases was treated with warmed CO2 of 37℃ ,and the control group of 24 cases was provided with room temperature C02. Body core temperature,intraoperative blood loss,blood oxygen saturation,heart rate,and postoperative recovery were compared between two groups. Results There were no significant differences in preoperative body temperature, body temperature before the establishment of CO2 insufflations,length of operation,case number of low temperature happened after operation,heart rate more than 180 beats/min,blood oxygen saturation less than 90% and incision infection after surgery between two groups(P〉0.05). Before and after CO2 insufflations,the body core temperature was decreased by 0.70±0.28℃ in the observation group,while the control group was reduced by 1.24 ±0.23℃,and the difference was statistically signifieant(t=7.6949,P〈0.001). Meanwhile,intraoperative blood loss in the control group was more than that in the observation group,and the recovery time of intestinal function and hospital stay were longer in the control group,and the differences were statistically significant(P〈0.05). Conclusion In laparoseopie operation, using warmed CO2 insufflations can keep body temperature in the normal range and avoid postoperative hypothermia,and can reduce intraoperative blood loss,promote recovery of intestinal function,and reduce hospital stay.
