摘要
目的：分析利水活血温阳方对慢性心力衰竭患者心阳虚证的辨证治疗效果。方法：选取于2015年6月—2016年12月本院收治的60例慢性心力衰竭且辨证为心阳虚症的患者作为本次研究对象,按数字随机表法分为观察组和对照组。两组患者在入组后采取常规治疗,对照组服用氟伐他汀治疗;观察组在此基础上加服利水活血温阳方治疗。比较两组患者治疗后的临床疗效,并检测两组患者治疗前后左室舒张末期内径（LVEDD）、左室收缩末期内径（LVESD）和左室射血分数（LVEF）;检测血浆N末段脑钠肽（NT-pro BNP）、糖化血红蛋白A1c（Hb A1c）水平;对两组治疗后中医证候积分进行比较。结果：两组经治疗后观察组总有效率93.33%明显大于对照组总有效率66.67%,观察组临床疗效明显优于对照组（P〈0.05）。治疗后观察组左室舒张末期内径（LVEDD）、左室收缩末期内径（LVESD）明显低于对照组（P〈0.05）;左室射血分数（LVEF）明显高于对照组（P〈0.05）。两组患者NT-pro BNP、Hb A1C水平经治疗均明显下降,观察组治疗后NT-pro BNP、Hb A1C水平明显低于对照组（P〈0.05）。观察患者在接受治疗后,中医证候总积分为（0.72±0.06）分明显低于对照组（1.12±0.15）分（P〈0.05）。结论：利水活血温阳方对于心阳虚证的慢性心力衰竭患者具有良好的辨证治疗效果,能有效改善患者心脏功能和临床症状。
Objective： To analyze the effect of Lishui Huoxue Wenyang Decoction on patients with chronic heart failure of heart Yang deficiency sydnrome. Methods： Sixty patients with chronic heart failure who were admitted to our hospital from June 2015 to December 2016 were selected as the subjects of this study. The patients were randomly divided into observation group and control group. After the conventional treatment,the control group was treated with fluvastatin and the observation group took Lishui Huoxue Wenyang Decoction on the basis of the control group. The clinical effects of the two groups were compared. The left ventricular end diastolic dimension（ LVEDD）,left ventricular end systolic diameter（ LVESD） and left ventricular ejection fraction（ LVEF）were measured before and after treatment in two groups. The level of brain natriuretic peptide（ NT-pro BNP） and glycosylated hemoglobin A1 c（ Hb A1 c） in plasma N were measured. The two groups’ TCM syndrome integral after treatment was compared. Results： After treatment,the total effective rate of the observation group was significantly higher than that of the control group（ 93. 33% vs 66. 67%）. The observation group’ s was significantly better than that of the control group（ P 〈 0. 05）. After treatment,the left ventricular end diastolic dimension（ LVEDD） and left ventricular end systolic diameter（ LVESD） in the observation group were significantly lower than those in the control group（ P 〈 0. 05）. The left ventricular ejection fraction（ LVEF）was significantly higher than that in the control group（ P 〈 0. 05）. Two groups’ NT-pro BNP,Hb A1 C levels were significantly decreased after treatment. In the observation group after treatment,NT-pro BNP,Hb A1 C levels were significantly lower than those in the control group（ P 〈 0. 05）. After treatment,the total score of TCM syndrome（ 0. 72 ± 0. 06） in the observation group was significantly lower than the control group’s（ 1. 12 ± 0. 15）（ P 〈 0. 05）. Conclusion： Lishui Huoxue Wenyang Decoction for patients with chronic heart failure of heart Yang deficiency sydnrome has a good effect of treatment and can effectively improve the heart function and clinical symptoms.
