摘要
目的应用频域相干光断层深度增强成像技术观察重度非增殖期糖尿病视网膜病变全视网膜光凝术手术前后黄斑部脉络膜体积的变化。方法对2017年2~8月就诊本院的2型糖尿病视网膜病变患者24例(32眼)进行前瞻性对比分析,经荧光素钠血管造影(FFA)确诊为重度NPDR(观察组),并均接受PRP治疗。选取18例(32眼)正常眼作为对照组。PRP术前、术后1周及术后1、3个月,采用OCT的EDI技术对黄斑部进行水平扫描,获取MCV值。结果观察组术前MCV值明显低于对照组(P<0.05)。观察组术后1周和术后1个月MCV值均较术前增大,术后3个月MCV值较术前减小(P<0.05)。结论重度NPDR会导致脉络膜体积减小。PRP术后在短期内能够明显增加黄斑区脉络膜体积。EDI SD-OCT可用于评估NPDR患者的预后。
Objective To observe the changes of macular choroidal volume before and after panretinal photocoagulationin severe non-proliferative diabetic retinopathy by using depth-enhanced spectral domain optical coherence tomography. Methods 24 patients (32 eyes)with type 2 diabetic retinopath who were treated in our hospital from February toAugust 2017 were prospectively compared and analyzed. Severe NPDR(observation group) was confirmed by fluorescein sodium angiography(FFA), and all patients were given PRP treatment. 18 patients(32 healthy eyes) were selected as control group. Before PRP surgery, 1 week after surgery and 1 and 3 months after surgery, the macular areawas flat-scanned by OCT EDI technique to obtain the MCV value. Results The preoperative MCV value in the observation group was significantly lower than that in the control group (P〈0.05). The MCV value 1 week and 1 month aftersurgery in the observation group was increased compared with that before surgery. The MCV value was decreased 3months after surgery (P〈0.05). Conclusion Severe NPDR causes a decrease in choroidal volume. The macular choroidvolume can be significantly increased in a short term after PRP, and EDI SD-OCT can be used to assess the prognosisof NPDR patients.
