经后外侧入路采用meta钢板联合空心钉内固定治疗Letenneur Ⅲ型外侧Hoffa骨折 被引量:9

Meta plate and cannulated screw fixation for treatment of type Letenneur Ⅲ lateral Hoffa fracture through posterolateral approach

摘要 目的 :探讨经后外侧入路采用meta钢板联合空心钉内固定治LetenneurⅢ型外侧Hoffa骨折的疗效。方法:回顾性分析2012年6月至2016年6月采用经后外侧入路meta钢板联合空心钉内固定治LetenneurⅢ型外侧Hoffa骨折12例患者资料。男8例,女4例;年龄22~66岁,平均42岁;左侧5例,右侧7例。术前和术后随访均拍摄X线片及CT以评估骨折的位置和骨折愈合的情况。临床评价采用Letenneur评分标准。结果:12例患者切口均Ⅰ期愈合。骨折均骨性愈合,愈合时间11~32周,平均17.2周。术后未见内固定松动、移位及股骨髁坏死、下肢深静脉血栓发生。所有患者获随访,时间8~24个月,平均12.8个月。末次随访时采用Letenneur评分标准评价膝关节功能:优7例,良4例,可1例。结论:后外侧入路暴露容易,采用meta钢板联合空心钉内固定坚强可靠、骨折愈合率高、膝关节功能锻炼早、关节功能恢复好。 Objective: To explore clinical effect of meta plate with cannulated screw fixation in treating Letenneur Ⅲ lateral Hoffa fracture through posterolateral approach. Methods: From June 2012 to June 2016,12 patients with Letenneur Ⅲ lateral Hoffa fracture were treated with meta plate and cannulated screw fixation through posterolateral approach,including 8 males and 4 females;aged from 22 to 66 years with an average of 42 years old;and 5 cases on left side,7 cases on right side. Preoperative and postoperative X-ray and CT were taken before and after operation to assess location of fracture and fracture healing. Letenneur scoring standard was used to evaluate clinical effects. All patients were treated with meta plate combined with cannulated screws. Results: All incisions healed at stageⅠ. The fractures were healed from 11 to 32 weeks with an average of 17.2 weeks. No internal fixation loosening,displacement and necrosis of femoral condyle and deep vein thrombosis of lower extremity occurred. All patients were followed up from 8 to 24 months with an average of 12.8 months. At the latest follow-up,7 cases got excellent results,4 good and 1 moderate according to Letenneur scoring standard. Conclusion: Posterlateral approach is easy to expose,meta plate fixation with cannulated screws has advantages of strong internal fixation,high fracture healing rate,early knee joint function exercise and good recovery of joint function.
