丙泊酚与七氟烷预防不同剂量瑞芬太尼复合麻醉后痛觉过敏的效果比较 被引量:5

摘要 【目的】探讨并比较丙泊酚与七氟烷预防不同剂量瑞芬太尼复合麻醉后痛觉过敏反应(OIH)的效果。【方法】将80例择期腹腔镜胆囊切除术(LC)患者随机分为四组:丙泊酚+高剂量瑞芬太尼组(A1组)、丙泊酚+低剂量瑞芬太尼组(A2组)、七氟烷+高剂量瑞芬太尼组(B1组)、七氟烷+低剂量瑞芬太尼组(132),各20例。比较四组麻醉情况、瑞芬太尼总量、术后舒芬太尼消耗量及不良反应。【结果]四组患者手术时间比较差异均无显著性(P〉O.05),A1、A2组呼之睁眼时间、拔管时间长于B1、B2组(P〈O.05),B1组术后24h舒芬太尼累积消耗量多于其他三组(P〈0.05)其余组之间比较差异均无显著性(P〉0.05),A1组、B1组瑞芬太尼总量多于A2组、B2组,且A1组与B1组、A2组与B2组组间比较差异有显著性(P〈O.05);A2组术后6h、24h切口周围疼痛闽值及不同时间点视觉模拟评分(VAS)评分高于A1组、B1组、B2组,且A1组、B1组、B2组组间比较差异均有显著性(P〈O.05);四组不良反应发生率比较差异无显著性(P〉O.05)。【结论】与低剂量瑞芬太尼[O.1μg/(kg·min)]相比,高剂量[O.3μg/(kg·min)]更容易引发0IH;且丙泊酚减轻高剂量瑞芬太尼复合麻醉后0IH的效果优于七氟烷。
