摘要
目的探讨在瘢痕子宫人工流产术前采用米非司酮与米索前列醇预处理的临床疗效及安全性。方法选择接受人工流产手术的100例瘢痕子宫患者作为观察对象,随机分为研究组和对照组,每组各50例。研究组术前1天晚7点、手术当天早7点服用米非司酮25 mg;术前1小时阴道后穹窿塞用200μg米索前列醇。对照组术前采用利多卡因凝胶局麻。结果研究组宫颈软化程度(80%)优于对照组(36%),差异有统计学意义(P<0.01);研究组的手术使用时间及术中出血量少于对照组;两组术中人工流产综合反应的发生率比较,差异有统计学意义(P<0.05)。结论在瘢痕子宫患者人工流产术前采用米非司酮联合米索前列醇预处理,具有软化宫颈、减少术中出血量、缩短手术时间及减少人工流产综合反应发生的优点。
Objective To explore the clinical efficacy and safety of mifepristone and misoprostol pretreated before artificial abortion of scar uterus. Methods 100 cases of uterine cicatricial uterus were selected as the observation objects and they were randomly divided into study group and control group, 50 cases in each group. The study group took mifepristone 25 mg at 7：00 pm the first day before operation and 7：00 am early on the day of operation. 1 hour before the operation, 200 μg of misoprostol was used in the posterior fornix of the vagina. The control group was treated with lidocaine gel local anesthesia before operation. Results The degree of cervical softening in the study group（80%） was better than that of the control group（36%）, and the difference was statistically significant（P 〈 0.01）. The time of operation and the amount of bleeding in the study group were less than that of the control group. There was a significant difference in the incidence of artificial abortion in the two groups（P 〈 0.05）. Conclusion In the cases of pretreatment with mifepristone and misoprostol before an visible induced abortion in the early days of the scar uterine pregnancy,the conventional pretreatment in the early artificial abortion operation has the advantages such as softeningthe cervix,reducing the amount of bleeding during the operation,shortening the operation time and reducing the artificial abortion syndrome.
