摘要
目的探讨EZH2和Ki-67在宫颈癌组织中的表达及其临床意义。方法采用免疫组化法分别检测40例正常宫颈、62例宫颈上皮内瘤变(CIN)及174例宫颈癌组织中EZH2和Ki-67蛋白的表达,并分析EZH2和Ki-67与临床病理特征及预后的关系。结果宫颈癌组和CIN组中EZH2和Ki-67蛋白的阳性表达率明显高于对照组,且宫颈癌组中EZH2和Ki-67蛋白的阳性表达率明显高于CIN组。在宫颈癌组织中,EZH2和Ki-67与临床分期、淋巴结转移及浸润深度密切相关(P〈0.05),且EZH2蛋白与病理分级有关(P〈0.05)。在随访的166例宫颈癌患者中,EZH2和Ki-67蛋白阳性表达患者的无进展生存时间(PFS)和总生存时间(OS)明显短于EZH2和Ki-67阴性患者[PFS(月):(53.7±3.6)和(56.8±3.2)vs(62.8±2.4)和(64.3±2.1);OS(月):(56.2±3.0)和(58.7±2.8)vs(64.2±1.9)和(65.8±1.7),均P〈0.01]。COX比例风险模型多因素分析显示,临床分期、淋巴结转移、EZH2及Ki-67蛋白是影响宫颈癌患者预后的独立危险因素。结论 EZH2和Ki-67异常表达与宫颈癌临床病理特征及预后有一定的相关。
Objective To investigate the expression and clinical significance of EZH2 and Ki-67 in cervical carcinoma.Methods The expression of EZH2 and Ki-67 in 40 normal cervical tissues,62 cervical intraepithelial neoplasia(CIN) and 174 cervical cancer tissues were detected by immunohistochemistry.The relationships between the expression of EZH2 and Ki-67,clinicopathological characteristics of cervical cancer and the outcomes of patients were evaluated.Results The positive rates of EZH2 and Ki-67 protein in cervical cancer group and CIN group were both significantly higher than the control group.The positive rate of EZH2 and Ki-67 protein in cervical cancer group were significantly higher than CIN group.In cervical cancer tissues,EZH2 and Ki-67 were closely related to clinical stage,lymph node metastasis and depth of invasion(P〈 0.05),and EZH2 protein was also related to pathological grading(P 〈0.05).The progression free survival time(PFS) and overall survival time(OS) of patients with EZH2 and Ki-67 protein positive expression were significantly shorter than those negative patients,showed by the follow-up of 166 cervical cancer patients [PFS(month):(53.7±3.6) and(56.8±3.2)vs(62.8±2.4) and(64.3±2.1); OS(month):(56.2±3.0) and(58.7±2.8) vs(64.2±1.9) and(65.8±1.7),all P〈0.01].Multivariate analysis of COX proportional hazards model showed that,the clinical stage,lymph node metastasis,EZH2 and Ki-67 protein were independent risk factors for the prognosis of cervical cancer patients.Conclusion The abnormal expression of EZH2 and Ki-67 were related to the clinicopathological features and prognosis of cervical cancer.
