摘要
目的探讨头颈CT血管造影(CTA)、彩色多普勒血流成像(CDFI)及经颅多普勒(TCD)在缺血性脑血管病介入治疗前后的应用价值。方法选择2014年1月至2016年2月行血管内支架成形术治疗的缺血性脑血管病患者87例,所有患者治疗前后均行数字减影血管造影(DSA)、CTA、CDFI及TCD检查,以DSA检查为金标准,评价CTA、CDFI及TCD检查在介入治疗前后的应用价值。结果本研究中87例患者共检查883条颅内外血管,以DSA检查结果为金标准:术前CTA检查的准确性、敏感性和特异性依次为94.68%、89.27%和96.31%,术前CDFI检查的准确性、敏感性和特异性依次为95.58%、90.73%和97.05%,术前TCD检查的准确性、敏感性和特异性依次为93.77%、86.83%和95.87%;术后CTA检查的准确性、敏感性和特异性依次为94.90%、86.21%和95.52%,术后CDFI检查的准确性、敏感性和特异性依次为96.15%、91.38%和96.48%,术后TCD检查的准确性、敏感性和特异性依次为94.11%、81.03%和95.03%;CTA、CDFI及TCD检查结果与DSA检查结果的一致性满意(Kappa>0.75,P>0.05)。结论 CTA、CDFI及TCD均可作为缺血性脑血管病术前检查及术后评估的影像学手段。
Objective To explore the application value of head and neck CT angiography( CTA),color Doppler flow imaging( CDFI) and transcranial doppler( TCD) in patients with ischemic cerebrovascular diseases before and after interventional therapy. Methods Eighty-seven patients who were treated by endovascular stent plasty in our hospital from January 2014 to February 2016 were enrolled in the study. All the patients received digital subtraction angiography( DSA),CTA,CDFI and TCD examination before and after treatment,then taking DSA examination results as gold standard to evaluate the application value of CTA,CDFI and TCD before and after interventional therapy. Results A total of 883 intracranial and extracranial blood vessels in 87 patients were examined. Taking DSA examination results as gold standard,the accuracy,sensitivity and specificity of CTA before operation were 94. 68% ,89. 27% and 96. 31% ,respectively,which by CDFI examination were 95. 58% ,90. 73% and 97. 05% ,respectively,moreover,which by TCD were 93. 77% ,86. 83% and 95. 87% ,respectivelt,however the accuracy,sensitivity and specificity of CTA after operation were 94. 90% ,86. 21% and 95. 52% ,respectively,which by CDFI were 96. 15% ,91. 38% and 96. 48% ,respectively,moreover,which by TCD were 94. 11% ,81. 03% and 95. 03% ,respectively. The examination results by CTA,CDFI and TCD had satisfactory consistency with those by DSA examination( Kappa〉 0. 75,P 〉0. 05). Conclusion The CTA,CDFI and TCD can be regarded as the imaging examination means in preoperative examination and postoperative evaluation for patients with ischemic cerebrovascular diseases.
