摘要
输尿管支架是泌尿外科常用的器械之一,其临床疗效值得肯定,但也存在一系列的并发症或不足,包括置管后感染、支架皮壳形成、置管后不适、膀胱输尿管尿液反流、置管后遗忘、内镜操作二次取管等。可降解输尿管支架可以由PLA(poly lactic acid)、PGA(poly-glycolic acid)、SR-PLGA(self-reinforced poly lactic-glycolic acid)或SR-PLA(self-reinforced poly-L,D-lactide acid,SR-PLA 96)等材料制成,具有生物相容性良好、可降解、避免二次侵袭性操作等优点,有望取代现行的硅胶和聚氨酯输尿管支架。现已研制成型的支架包括自增强双螺旋短支架、喇叭状螺旋短支架、纺织型的薄壁复合纤维支架、Uriprene输尿管支架、具有纳米结构的电纺可降解输尿管支架、可降解塑料输尿管支架、TUDS支架等。上述支架较双J管存在可降解、无需内镜操作二次拔管等优势,但同时也存在改进的空间。现本文就可降解输尿管支架的研究现况及临床前景综述如下。
Ureteral stent is one of the most commonly used devices in urology.Despite of positive therapeutic outcome,it has a series of complications or disadvantages,including stent-associated infections,encrustation,stentassociated discomfort,stent-associated urinary reflux,forgotten stent,second cystoscopy procedure to remove stent and so on.Degradable ureteral stents,which could consist of PLA,PGA,SR-PLGA or SR-PLA,can be biocompatible,degradable and free from second cystoscopy procedure,holding great promise to replace current silicone and polyurethane ureteral stents.There have been various degradable ureteral stents with several advantages over traditional double J stent,however,further research is still necessary.
