摘要
目的:分析对接受腹腔镜胆囊切除术的患者在手术结束前使用罗哌卡因进行术后镇痛的效果。方法 :将2015年10月至2016年10月期间在梓潼县中医医院进行腹腔镜胆囊切除术的94例患者作为本研究的对象。随机将这94例患者分为罗哌卡因组(n=47)和布比卡因组(n=47)。对两组患者均进行腹腔镜胆囊切除术。在手术结束前,使用罗哌卡因对罗哌卡因组患者进行术后镇痛,使用布比卡因对布比卡因组患者进行术后镇痛,然后比较两组患者术后各时间点的疼痛评分、额外镇痛药物的使用率、额外镇痛药物的使用次数、术后并发症的发生率及其生活质量的评分。结果 :术后2 h、6 h、12 h及24h,罗哌卡因组患者切口、肩背部和内脏疼痛的评分均低于布比卡因组患者,差异有统计学意义(P<0.05)。罗哌卡因组患者额外镇痛药物的使用率和术后并发症的发生率均低于布比卡因组患者,其额外镇痛药物的使用次数少于布比卡因组患者,差异有统计学意义(P<0.05)。罗哌卡因组患者术后生理机能、躯体健康、饮食状况及心理状态的评分均高于布比卡因组患者,差异有统计学意义(P<0.05)。结论 :对接受腹腔镜胆囊切除术的患者在手术结束前使用罗哌卡因进行术后镇痛的效果显著,能有效地减轻其术后的疼痛感,降低其术后并发症的发生率,提高其生活质量。
Objective： To analyses effect of applying ropivacaine in the end of operation for analgesia for patients accepted laparoscopic cholecystectomy. Methods： Select 94 cases of patients accepted laparoscopic cholccystectomy in Traditional Chinese Medical of Zitong county from October 2015 to October 2016 for research object. Randomly divided into ropivacaine group（n=47）and bupivacaine group（n=47）.Treat 22 groups with laparoscopic cholecysteetomy. After operation, apply ropivacaine for postoperative analgesia for ropivacaine group, apply bupivacaine for postoperative analgesia for bupivacaine group. Then compare pain score in each timing, utilization of other anesthetic, usage count of other anesthetic, incidence of postoperative complications and life quality score of 2 groups. Results：In 2h,6h,12h and 24h, Shoulder- back and viscera pain score of ropivacaine group are lower than that in bupivacaine group （ P 〈 0.05 ） . Utilization of other anesthetic and incidence of postoperative complications of ropivacaine group are lower than that in bupivacaine group, usage count of other anesthetic of bupivacaine group is less than that in bupivacaine group （ P 〈 0.05 ） . The scores of Physiological function , Physical Health, food intake and psychological state are higher than that in bupivacaine group （ P 〈： 0.05 ） . Conclusion： Effect of applying ropivaeaine in the end of operation for analgesia for patients accepted laparoscopic cholecystectomy is significant, ropivacaine can alleviating pain,decrease incidence of postoperative complications, increase life quality of patients.
