摘要
目的探讨不同时机探查干预预防重度宫腔粘连分离术后患者粘连复发的临床效果。方法选取无锡市中医医院2014年3月—2016年8月收治的重度宫腔粘连患者95例,根据分离术后探查放置圆环的时间点及持续时间不同将所有患者分为观察组(n=45)和对照组(n=50)。患者均于宫腔镜下行重度宫腔粘连分离术,观察组患者于术后放置圆环,同时进行3个月的药物治疗后再次行宫腔镜检查取环。对照组患者先取环,3个月后再次行宫腔镜检查。比较两组患者术后6个月、1年后的预防效果、宫腔粘连复发情况、月经情况。结果术后6个月、1年,观察组患者预防效果均高于对照组(P<0.05)。术后6个月、1年,观察组患者月经改善率高于对照组,宫腔粘连复发率低于对照组(P<0.05)。结论对于重度宫腔粘连患者,采取术后放置圆环的同时进行药物治疗3个月后,再次宫腔镜检查取环的治疗方案和取环时机治疗效果好,复发率低。
Objective To investigate the clinical efficacy of different time point intervertebral exploration for prevention of adhesions recurrence after severe intrauterine adhesions separation. Methods A total of 95 cases of patients with severe intrauterine adhesions were selected from March 2014 to August 2016 in Wuxi Traditional Chinese Medicine Hospital,which were divided into observation group（ n = 45） and control group（ n = 50） according to different time points intervertebral exploration. All patients were underwent severe intrauterine adhesions separation,the observation group was placed the ring after operation,and the hysteroscopy was taken again for three months after the drug treatment. Control group was taken the ring,and took hysteroscopy after three months. The prevention of adhesions recurrence,adhesions recurrence and menstrual conditions after 6 months,1 year of operation were compared between the two groups. Results After 6 months,1 year of operation,the overall effective rate of observation group were higher than control group（ P 〈 0. 05）. After 6 months,1 year of operation,the observation group of menstruation improvement rate was higher than control group,uterine adhesions recurrence rate was lower than control group（ P 〈 0. 05）. Conclusion As for severe intrauterine adhesions patients,placing the ring after operation,and the hysteroscopy was taken again for three months after the drug treatment have a better clinical effect,and with lower recurrence rate.
