床边深度呼吸训练可改善晚期帕金森病病人吸气肌功能和日常生活能力 被引量:11

Bedside deep breathing exercises can improve the inspiratory muscle function and ability of daily living of patient with advanced Parkinson's disease

摘要 目的探讨床边深度呼吸训练对晚期帕金森病(Parkinson’s disease,PD)病人吸气肌功能和日常生活能力的治疗效果。方法利用Breath-link呼吸功能评估与训练系统,分别对16例晚期PD病人与18例正常对照进行吸气功能测定和比较。同时16例PD病人随机分为深度训练组(行床边深度呼吸训练)和常规训练组(行常规呼吸训练),训练前和训练1月后测定吸气肌功能,采用疲劳严重程度量表(fatigue severity scale,FSS),6分钟步行试验及改良Barthel指数量表(MBI)评定各组训练前后病人呼吸功能、疲劳严重程度、6分钟步行距离(6MWD)及日常生活自理能力改善情况。结果 2组训练前吸气肌功能、FSS评分、6MWD及MBI评分差异无统计学意义(P>0.05);训练后2组PD病人最大吸气阻力、最大吸气流速、FSS评分、6MWD和MBI评分均较入组前明显改善,且深度训练组改善程度显著优于常规训练组(P<0.05)。结论对严重吸气功能障碍的晚期PD病人实施床边深度呼吸训练,能提高其吸气功能,改善疲劳和日常生活活动能力。 Objective To investigate the effect of bedside deep breathing exercises on the inspiratory muscle function and ability of daily living in patients with advanced Parkinson 's disease( PD). Methods The inspiratory function of sixteen advanced PD patients and eighteen normal subjects were measured by Breath-link respiratory function assessment and training system. Sixteen advanced PD patients were randomly divided into the deep breathing training group and the conventional training group. The inspiratory muscle function,fatigue severity,six minute walking distance( 6 MWD),activities of daily living were evaluated by Breath-link respiratory function assessment and training system,the fatigue severity scale( FSS),six-minute walking test( 6 MWT) and the modified Barthel index( MBI) respectively before and after intervention. Results After breathing training,every indexs of the conventional training group and deep breathing training group were significantly improved than before( P〈0. 05). Maximal inspiratory resistance( 41. 65 ± 17. 21 cm H2 O),peak inspiratory flow rate( 2. 94±1. 35 L/s),FSS score( 5. 04±2. 86 points),6 MWD( 312. 00±98. 75 m) and MBI score( 64. 89±20. 43 points) of deep breathing training group were significantly improved than those of conventional training group after training( P〈0. 05). Conclusions The advanced PD patient with severe inhalation dysfunction can take advantage of bedside depth breathing training to improve their inspiratory function,fatigue and daily living activity significantly.
