摘要
目的:探讨超声引导下颈部肿大淋巴结穿刺活检诊断的临床效果。方法:随机选取2015年4月至2016年4月我院行超声引导下颈部肿大淋巴结穿刺活检的75例患者为研究对象,回顾性分析其临床资料。结果:75例患者共取材240针,其中222针复合病理检查要求,穿刺活检成功率达92.5%,所有患者均无明显不良反应,颈部按压后基本无出血。穿刺活检样本活检结果显示74例患者诊断明确,1例诊断不明,后经术后组织学检查证实为神经鞘瘤。诊断明确的患者中淋巴结恶性病变患者占70.7%,其中淋巴瘤10例,转移癌43例;良性病变共22例,占29.3%,主要为淋巴结结核和淋巴炎。以术后病理检查结果为准,超声诊断和超声引导穿刺活检显示良性病变准确率分别为77.3%和97.7%,恶性病变准确率分别为93.4%和100%,整体符合率分别为88.7%和99.3%;超声引导穿刺活检诊断准确率显著高于超声检查(P<0.05)。结论:超声引导下颈部肿大淋巴结穿刺活检诊断临床效果满意。
Objective： To evaluate the clinical effect of ultrasound-guided cervical lymph nodes biopsy on diagnosis. Methods： Randomly selected 75 cases from April 2015 to April 2016 in our hospital for ultrasound-guided biopsy of the cervical lymph nodes,and clinical data were retrospectively analyzed. Results： A total of 240 strip of tissue were taken from 75 patients,of which 222 strips fulfilled pathological requirements,and the success rate of puncture biopsy was 92. 5%. All patients had no obvious adverse reactions,and there was nearly no bleeding after compression. Pathology showed 74 cases were diagnosed correctly,1 case was not determined,until histological examination after operation confirmed it schwannoma. Among those diagnosed patients,70. 7% were lymph node malignant disease,of which 10 were lymphoma and 43 were metastatic carcinoma; 22 cases were benign lesions,accounting for 29. 3%,mainly for lymphoid tuberculosis and lymphadenitis. The accuracy of ultrasonography and ultrasound-guided biopsy in diagnosis of benign lesions subjected to pathological reports was 77. 3% and 97. 7%,and accuracy of malignant lesions was 93. 4% and 100% respectively. The overall diagnostic accuracy was 88. 7% and 99. 3%,and the accuracy of ultrasound-guided biopsy was significantly higher than that of ultrasonography（P〈0. 05）. Conclusion： The clinical effect of ultrasound-guided cervical lymph node biopsy in diagnosis is satisfactory,which is suitablefor a wide range of clinical application.
