摘要
目的分析云南省内首起输入性人感染H7N9禽流感聚集性病例的流行病学调查结果,总结人感染H7N9禽流感病例的流行病学特征,为今后防控人感染H7N9禽流感病例及流行病学调查提供参考。方法回顾性描述、分析云南省首起输入性人感染H7N9禽流感聚集性病例流行病学调查过程、密切接触人员采样检测结果及外环境监测采样结果,总结人感染H7N9禽流感病例的流行病学特征。结果云南省首起人感染H7N9禽流感疫情中的确诊病例为输入性病例,病例乙是病例甲的密切接触者,2例病例为母女关系,病例乙较病例甲晚6 d发病,具有禽类共同暴露史,属于家庭聚集性病例,2例病例均已死亡。医学观察期间44名密切接触者中1人出现流感样症状,经实验室检测,流感病毒通A、通B均为阴性,其余密接人员皆无异常。结论此次疫情为云南省首起输入性人感染H7N9禽流感疫情,初步判定为共同暴露聚集性疫情,传播途径可能为禽-环境-人或禽-人,不排除人传人可能,整个处置过程为今后类似事件处置提供了宝贵的参考。
[Objective]To analyze the epidemiological investigation results of the first clustered cases of imported human infection with avian influenza A(H7 N9) in Yunnan Province,summarize the epidemiological characteristics of human infections with avian influenza A(H7 N9),and provide the reference for further prevention and control,and epidemiological investigation.[Methods]The epidemiological investigation process of the first clustered cases of imported human infection with avian influenza A(H7 N9) in Yunnan Province,as well as detection results of samples from close contacts and results of external environment monitoring were retrospectively described and analyzed,to summarize the epidemiological characteristics of human infections with avian influenza A(H7 N9).[Results]The first confirmed cases of human infection with avian influenza A(H7 N9) in Yunnan Province were imported,case B was the close contact of case A,and they are mother-daughter relationship. The case B had been attacked six days after the onset of the case A. They have the common exposure history to poultry,which belonged to familial clustering case. Both two cases have died. 1 people had appeared flu-like symptoms among 44 close contacts during the period of medical observation. The laboratory test showed that both influenza virus A and B were negative,and other close contacts had no abnormal symptoms. [Conclusion]The epidemic situation is the first clustered cases of imported human infection with avian influenza A(H7 N9) in Yunnan Province,which is preliminarily judged to be a clustered event with common exposure history.The transmission rout may be poultry-environment-human or poultry-human,and the human-to-human transmission does not be excluded. The whole disposal process provides a valuable reference for future similar events.
