腹部异位胰腺的CT诊断临床分析 被引量:1

摘要 目的探讨螺旋CT诊断腹部异位胰腺的应用价值。方法选取我院收治的经手术病理诊断确诊为腹部异位胰腺患者5例为研究对象,所有患者均在入院后行多层螺旋CT诊断,回顾分析患者的临床资料,并对其CT平扫、增强扫描时的影像学表现进行分析。结果经多层螺旋CT诊断,5例异位胰腺患者的发病位置分别位于胃体大弯侧(1例),胃窦小弯侧(2例)、十二指肠降部(1例)、空肠上段系膜区(1例),均为单发病变,病变主要生长于胃肠道黏膜下层,呈腔内生长。其中,3例患者病灶边缘光滑、呈类圆形;1例患者病灶形态不规则,合并溃疡;1例患者病灶边缘有轻度分叶,呈不规则状;胃肠道病灶大小在0.8 cm×2.1 cm^2.0 cm×2.8 cm间,位于肠系膜病灶的大小为2.5cm×2.9 cm。胃肠道病变中1例患者病灶周围有脂肪密度出现,1例肠系膜病变患者病灶周围存在大片渗出。经常规CT平扫,3例患者的病灶密度均匀,增强扫描时各期强度程度与主胰腺强化一致,其中1例患者存在中央导管征;1例合并溃疡患者病灶平扫密度欠均匀,增强扫描后动脉期呈不均匀明显强化,静脉期及延迟期病灶大部分区域与主胰腺强化程度相似;1例合并胰腺炎的患者平扫时病灶密度稍低,强化程度也低于主胰腺。结论多层螺旋CT可以对腹部异位胰腺患者的病灶大小、位置、形态以及平扫、增强扫描时的密度变化特点进行充分反映,为腹部腹部异位胰腺的诊断提供可靠依据,值得临床推广应用。 Objective To investigate the value of spiral CT in the diagnosis of abdominal heterotopic pancreas. Methods Five patients heterotopic pancreas in the abdomen who diagnosed by pathology in our hospital as the research object, all patients on admission underwent multislice CT diagnosis, analysed of clinical data of the patients, and the CT scan, enhanced scan imaging analysis. Results the multi-slice spiral CT diagnosis of 5 cases of ectopic pancreas, the location of disease patients were located in the greater curvature of the gastric body side ( 1 case), gastric juice side (2 cases), descending duodenum (1 case), mesangial jejunum (1 cases), were solitary lesions, lesions mainly grow in gastro-intestinal tract the submucosa, a cavity growth. Among them, 3 cases were round, smooth edge; 1 case had irregular shape, 1 case of patients with ulcer, the lesion edge had mild leaf, irregular shape; gastrointestinal lesion size between 0.8 cm-2.1 cm to 2.0 crux2.8 cm, located in the mesentery of size 2.5 cm-2.9 cm lesions. In the gastrointestinal lesions, 1 case had fat density around the lesion, and there was a mass of exudation around the lesion in 1 mesenteric lesions. Routine CT scan, 3 cases were homogeneous density, enhanced the strength and the degree of consistency the main pancreatic enhancement scan, 1 case of patients with central catheter syndrome; 1 case with ulcer lesions in patients with heterogeneous density scan, enhanced scan showed inhomogeneous enhancement in arterial phase, venous phase and most of the area the delay period and the main pancreatic lesion enhancement degree of similarity; 1 case complicated with pancreatitis patients with unenhanced lesions slightly lower density, degree of enhancement was also lower than the main pancreatic. Conclusion Muhislice spiral CT in patients with abdominal ectopic pancreatic lesion size, location, morphology and enhanced CT density changes were fully reflected the characteristics of scanning time, and provide a reliable basis for the diagnosis of abdominal EP, worthy of clinical application.
