摘要
李士懋教授运用平脉辨证思辨体系治疗痿痹病疗效突出,在寒证痿痹的临证思辨过程中首分虚实,以脉解症,以脉解舌,进而分析病机、确立治法和方药。虚寒者,脉多弦拘而弱或少力,治以扶正补虚为主,滋养脏腑为辅,可加大寒痉汤温补之品的剂量;实寒者,脉多弦拘紧而紧实,沉取有力,治宜祛邪和络。并通过医案说明治疗痿痹的平脉辨证思路和选方用药特点。
The effect of Professor LI Shimao treating flaccidity-arthralgia by using syndrome differentiation based on feeling pulse system is remarkable. In the process of clinically differentiating flaccidity-arthralgia of cold syndrome,differentiating deficiency or excess syndrome is the first. Pulse manifestation is used to explain symptoms and tongue manifestation. Then pathogenesis is analyzed first,before the method and prescription-herbs are determined. The pulse of patients with deficiency-cold syndrome is mostly wiry,spasm and weak or less powerful. The treatment is mainly reinforcing vital Qi and tonifying deficiency,with nourishing Zang-fu viscera. The dose of warming-nourishing herbs in Hanjing Decoction（ 寒痉汤） could be increased. The pulse of patients with excess-cold syndrome is mostly wiry,spasm,firming and powerful when taking heavily. The treatment should be eliminating pathogenic factors and harmonizing collaterals. The thoughts of syndrome differentiation based on feeling pulse and the characteristics of selecting prescription-herbs are explained by medical records.
