摘要
目的探讨对比传统中草药与单纯西药利尿剂对糖尿病肾病的治疗效果。方法根据患者自愿原则,将本院中医门诊符合糖尿病肾病水肿诊断标准的患者按3:1的比例随机分为对照组、治疗组。对照组(37例):限制饮食,根据病情分别给予糖适平、诺和龙、胰岛素等降血糖药,以及降血脂、降血压、利尿、白蛋白输入;肾功能不全者,中药保留灌肠每周2~4次。治疗组(107例):在对照组治疗的基础上一般不输白蛋白,改用中药汤剂。观察比较两组的疗效。结果治疗组与对照组的总有效率分别为93%与50%,治疗组明显优于对照组(P<0.05)。治疗组与对照组血浆蛋白升高者分别占95.9%与71.4%,24小时尿蛋白减少1g以上者分别占96.2%与45.7%,治疗组明显优于对照组(P<0.05)。结论运用好中草药温阳益气、活血行气、祛湿利水的功效,对肾性水肿及糖尿病其他并发症都有很好的辅助作用,治疗效果明显优于单纯西医治疗。
Objective Diabetic nephropathy often causes edema,especially moderate to severe ones,in the late stage of diabetic nephropathy,which brings out a large number of proteinuria,low level of plasma protein,high blood pressure,and renal insufficiency.Western medicine diuretics alone act mildly or invalidly to this edema.This paper is to investigate the effectiveness of treatment through comparing the traditional Chinese herbal medicine and pure western medicine diuretics.Method According to the voluntary principle,patients with diabetic nephropathy edema in our hospital,were randomly divided into control group and treatment group according to the proportion of 3：1.The control group（37 cases）：restricted diet,given Glurenorm,Novonorm insulin and so on according to their blood glucose level,and at the same time fall hematic fat,fall blood pressure,diuresis,input albumin;people who suffers from renal insufficiency,were given Chinese medicine retain enema 2 to 4 times a week.The treatment group（107 cases）：Instead of inputting albumin,patients of the treatment group took traditional Chinese medicinal broth,on the basis of the treatment of the control group.Then observed the curative effect of two groups.Result Total effective rate in treatment group and control group respectively is 93% and 50%,the treatment group was better than control group（P〈0.05）.Rate in increaing plasma protein respectively is 95.9% and 71.4%.More than 1g/24 h Urinary protein decreaing in the treatment group and control group is 96.2% and 45.7% respectively,the treatment group was better than control group（P〈0.05）.Conclusion The author thinks that,Chinese herbal medicine can warm and nourish yangqi,invigorate the circulation of blood,clearing damp and play a supporting role in the treatment of the renal edema and other complications.The effect of this treatment is better than pure western medicine treatment.Combination of the Western Medicine and Traditional Chinese Medicine is the inevitable trend in the future to treat various incurable diseases.Precise treatment in western medicine with the whole regulation of traditional Chinese medicine will obtain very good clinical effect.
