经皮椎体成形术联合唑来膦酸治疗绝经后骨质疏松性椎体骨折的临床效果观察 被引量:8

Clinical effects of percutaneous vertebroplasty combined with zoledronic acid in the treatment of postmenopausal osteoporotic vertebral fracture

摘要 目的:探讨经皮椎体成形术(PVP)联合唑来膦酸治疗绝经后骨质疏松性椎体骨折的临床效果。方法:选取2015年5月至2016年6月江苏省连云港市第一人民医院收治的绝经后骨质疏松性椎体骨折患者72例,按随机数字表法将其分为对照组和研究组,每组36例。对照组实施PVP术治疗,研究组实施PVP术联合唑来膦酸治疗。比较两组患者治疗前、后骨密度(BMD)、骨转换生化指标、髋关节强度指数(FSI)、日常生活能力(AQL)评分及疼痛视觉模拟评分(VAS)。结果:治疗后6个月,研究组VAS评分、血清骨特异性碱性磷酸酶(s BAP)水平明显低于对照组(P<0.05),而血清Ⅰ型前胶原羧基末端前肽(PICP)水平及Wards三角、股骨颈及大粗隆BMD值均显著高于对照组(均P<0.05);研究组的FSI和AQL评分明显高于对照组(P<0.05),椎体新发骨折发生率明显低于对照组(P<0.05)。结论:PVP联合唑来膦酸治疗绝经后骨质疏松性椎体骨折能有效减轻患者疼痛,增加BMD、FSI,同时还可降低骨转换率,预防椎体新发骨折,提高患者生活质量,值得临床推广应用。 Objective: To investigate the clinical effects of percutaneous vertebroplasty (PVP)combined with zoledronic acid in the treatment of postmenopausal osteoporotic vertebral fractures (POVF). Methods: 72 patients with POVF admitted in our hospital from May 2015 to June 2016 were selected and randomly divided into a study group and a control group, with 36 cases in each group. The patients in the study group were treated with PVP combined with zoledronic acid, and those in the control group were treated with PVP only. Bone turnover biochemical indexes, bone mineral density (BMD), hip strength index (FSI), the activities of daily life (AQL) scores and visual analogue scale (VAS)score were compared. Results: After 6 months of treatment, The VAS score and serum bone-specific alkaline phosphatases (BAP)level in the study group were significantly lower than those in the control group (P〈 0. 05), while the carboxyterminal propeptide of type I procollagen (PICP) level and the BMD of Wards triangle, femoral neck and greater trochanter were significantly higher (P~ 0. 05). The FSI and AQL scores of the study group were significantly higher than those of the control group (P〈 0. 05 ). The incidence of new fractures in the study group was significantly lower than that in the control group(P〈0. 05). Conclusion: PVP combined with zoledronic acid could effectively relieve pain, increase BMD and FSI, reduce bone turnover rate, prevent the occurrence of new vertebral fracture and improve the life quality of patients with POVF.
