摘要
目的研究机器通气患者的应用支持方法 ,并探究肠内营养与肠外营养的临床使用效果。方法选取2016年3月~2017年3月我院收治的80例机械通气患者,因呼吸衰竭在我院ICU接受治疗,采用随机方法分为对照组和观察组各40例。对照组实施肠外营养支持,观察组实施肠内营养支持。对血液中的清蛋白、前清蛋白、血红蛋白,分别在支持前后进行观察,观察两组撤机时间。结果撤机时间,观察组短于对照组,差异有统计学意义(P<0.05);两组患者血液中清蛋白以及其他相关指标在支持前1 d显差异无统计学意义(P>0.05),支持后28 d观察组则改善更佳。对照组28 d内撤机成功率为70.0%,观察组28 d内撤机成功率为92.5%,差异有统计学意义(P<0.05)。结论机械通气患者给予肠内营养相比于肠外营养,更有利于患者吸收利用,缓解机械通气带来的能量消耗,更符合患者的生理状态,在减少患者通气时间的同时,也可以显著的减轻患者的痛苦,降低患者住院的费用,减轻患者和家属的负担,因此肠内营养值得临床优先选择使用。
Objective To study the application of support methods in patients receiving mechanical ventilation,and to explore the clinical application effect of enteral nutrition and parenteral nutrition.Methods 80 patients receiving mechanical ventilation from March 2016 to March 2017 were admitted.The patients were all affected with respiratory failure and received treatment in ICU of our hospital;the patients were divided into the control group and the observation group by randomization.The control group was treated with parenteral nutrition support,and the observation group was given enteral nutrition support.The albumin and prealbumin,hemoglobinin the blood were observed.1 d before intervention and 28 days after intervention.The machine withdrawal time was observed simultaneously in both groups.Results In terms of machine withdrawal time,the observation group was shorter than the control group,there were significant differences between the two groups(P〈0.05);there was no significant difference in serum albumin and other related indexes between the two groups before intervention(P〉0.05).28 d after intervention,the improvement in the observation group was better than the control group.The success rate of withdrawal in the control group was 70.0% within 28 days,and in the observation group was,the difference was statistically significant(P〈0.05).Conclusion In the patients with mechanical ventilation,compared with parenteral nutrition,the enteral nutrition is more beneficial to the patients′ absorption and utilization,alleviating the energy consumption caused by mechanical ventilation,which is more in accordance with the physiological state of the patients.In reducing the patient ventilation time,it can also significantly reduce the suffering of patients,reduce the cost of hospitalization,and reduce the burden on patients and their families.Therefore,enteral nutrition is worthy of prioritized clinical application.
