摘要
甲状腺癌是头颈部最常见的恶性肿瘤。近年来,甲状腺癌的发病率呈逐年上升趋势。腔镜甲状腺手术在近20年得到了迅速发展。腔镜下淋巴结清扫作为甲状腺癌根治术的重要组成,要求术者有丰富的开放手术经验及娴熟的腔镜技巧。熟练掌握超声刀的应用技巧,熟悉清扫范围的解剖关系,对于避免神经、甲状旁腺等结构损伤具有重要作用。术中神经监测(IONM)、纳米碳甲状旁腺负显影技术等新器械、新技术的应用使腔镜下操作越来越安全可靠。
Thyroid carcinoma is the most common malignant tumor in neck and head. The incidence of thyroid carcinoma is increasing in recent years. Endoscopic thyroidectomy(ET) has developed rapidly within twenty years. As an important part of radical thyroidectomy, lymph node dissection via endoscopy demands rich experience of open surgery and endoscopic skill. It is important to know the anatomy of dissection area to avoid injury of nerves, vessels and parathyroid glands. The use of IONM and carbon nanoparticles facilitates the procedure.
