摘要
目的探讨不同胎龄早产儿早期凝血功能指标差异,为早产儿临床诊治提供依据。方法选取2015年1月-2016年1月该院出生的165例早产儿为研究对象,按照胎龄将其分为早期早产儿44例,中期早产儿50及晚期早产儿71例;另选取同期足月新生儿52例作为对照组;于出生后2 h内检测其凝血功能指标。比较各组患儿及产妇临床资料与凝血功能指标。结果3组不同胎龄早产儿凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)及凝血酶时间(TT)比较,差异均无统计学意义(均P>0.05);PT、APTT 95th%以晚期早产儿略低于早期及中期早产儿,TT随周龄增大出现减小趋势,但差异无统计学意义(P>0.05);足月新生儿PT、APTT、TT较3组早产儿均显著下降(P<0.05),FIB则显著升高(P<0.05);4组产妇PT、APTT、FIB、TT比较差异均无统计学意义(均P>0.05)。结论与足月新生儿相比,早产儿存在早期凝血功能降低,检测PT、APTT、FIB、TT水平可以早期发现凝止血功能异常,为早产儿的早期干预、临床指导用药提供依据。
Objective To explore the differences of coagulation function indexes in preterm infants at different gestational ages,provide a basis for clinical diagnosis and treatment of premature infants. Methods A total of 165 premature infants born in the hospital from January2015 to January 2016 were selected as study object,then they were divided into early preterm infant group( 44 infants),middle preterm infant group( 50 infants),and late preterm infant group( 71 infants). Fifty-two full-term neonates during the same period were selected as control group. The blood coagulation indexes were detected within two hours after birth. The clinical data and blood coagulation indexes were compared among different groups. Results There was no statistically significant difference in prothrombin time( PT),activated partial thromboplastin time( APTT),fibrinogen( FIB),and thrombin time( TT) among early preterm infant group,middle preterm infant group,and late preterm infant group( all P〈0. 05). PT and APTT in late preterm infant group was slightly lower than that in early preterm infant group and middle preterm infant group,TT showed decreasing trend with gestational week,but there was no statistically significant difference( P〈0. 05). PT,APTT,and TT in control group was statistically significantly lower than that in the other three groups( P〈0. 05); FIB in control group was statistically significantly higher than that in the other three groups( P〈0. 05). There was no statistically significant difference in PT,APTT,FIB,and TT among the pregnant women in the four groups( P〈0. 05). Conclusion Compared with full-term neonates,coagulation function of preterm infants decreases,coagulation dysfunction can be found early by detection of PT,APTT,FIB,and TT,the study provides a basis for early intervention and clinical treatment of premature infants.
