甘草酸制剂相关低钾血症主动监测评价研究 被引量:1

摘要 目的探讨甘草酸制剂相关低钾血症主动监测评价的技术方法。方法依托运行良好的医院管理信息系统(HIS),利用触发器技术,设计可主动监测评价甘草酸制剂相关低钾血症的软件。以人工病例回顾性研究为对照,对2014年6月1日至2015年5月31日所有住院患者进行监测,评价主动监测评价系统的监测效果。结果主动监测评价系统具有监测、排除、评价多种功能,能主动监测患者的血钾值,并对血钾降低的导致因素进行分析,对低钾血症的发生是否与甘草酸制剂相关作出评价,暴露相关药品不良反应。与人工病例回顾性研究结果相比,主动监测评价系统病例筛查速度是人工的5 127.27倍;相关不良反应数量是实际报告数量的122.5倍;一致性为93.0%,Kappa值等于0.647;主动监测评价系统显示了较好的可行性和一致性。结论主动监测评价系统能扩大监测范围,提高检测效率,节省人力和时间,降低漏报率,提示基于HIS的药品不良反应主动监测评价技术确实可行,应用前景良好。 Objective To investigate the technology methods of active monitoring of hypokalemia related to glycyrrhizic acid preparation.Methods The software for active monitoring and evaluation of hypokalemia related to glycyrrhizic acid preparation was designed by well-functioning HIS and trigger technology.A retrospective study of artificial cases was selected as the control,all hospitalized patients were monitored from June 2014 to May 2015 to evaluate the monitoring effect of active monitoring and evaluation system.Results The active monitoring and assessment system had several functions of monitoring,excluding,evaluating,and so on.It could take the initiative to monitor the potassium value of the patients,and analyze the factors of the decrease in serum potassium.Also the system could evaluate the relationship between the occurrence of hypokalemia and glycyrrhizic acid preparation,so that the relevant ADR could be exposed.Compared with the results of artificial case retrospective studies,the cases of screening speed of the system was 5127.27 times that of the artificial.The report number was 122.5 times that of the artificial.The consistency between the two was 93.0%,and the kappa value was 0.647.The active monitoring and evaluation system showed good feasibility and consistency.Conclusion The active monitoring and evaluation system could expand the scope of monitoring,improve the testing efficiency,save manpower and time,and reduce the non-response rates.The active monitoring and evaluation technology based on HIS is feasible,and has good prospect of practical application.
