三维斑点追踪技术在冠心病早期左室收缩功能评价中的应用 被引量:2

摘要 目的 :探讨三维斑点追踪技术在冠心病早期左室收缩功能评价中的应用。方法 :选取50例冠心病早期患者作为观察组,选取同期健康对照50例作为对照组,2组均采用三维斑点追踪技术,检测左室整体长轴应变(GLS)、径向应变(GRS)、圆周应变(GCS)、面积应变(GAS)和其他参数,并比较2组各项指标。结果:观察组左心室舒张期容积、左心室收缩末期容积显著高于对照组,左室射血分数、每搏量显著低于对照组(均P<0.05),而心输出量、左室收缩末期质量、球形指数比较差异无统计学意义(P>0.05);在GLS、GCS、GRS、GAS水平上,观察组均低于对照组(P<0.05),单支有侧支循环组>单支无侧支循环组>多支有侧支循环组>多支无侧支循环组,组间比较差异均有统计学意义(均P<0.05)。结论:三维斑点追踪技术能发现冠心病早期左室收缩功能情况,对判断冠心病严重程度和预后有指导价值。 Objective:To investigate the clinical diagnosis and treatment of left ventricular systolic function in patients with early stage of coronary heart disease by three dimensional speckle tracking imaging. Methods:50 cases of coronary heart dis-ease patients were selected,as the observation group,selected 50 cases of healthy control in the same period as control group, were treated by three-dimensional speckle tracking,detection of left ventricular longitudinal strain,radial strain,circumferential strain,area strain and other parameters,compared with the index parameters. Results:the observation group in LVEDV,LVESV was significantly higher than the control group,while in LVEF,SV were significantly lower than the control group,there was sig-nificant difference between two groups (P〈0.05),while there was no significant difference in CO,LVESM,SPI (P〉0.05);GLS, GCS,GRS in the observation group,the level of GAS were lower than the control group (P〈0.05),single collateral circulation grou P〉single no collateral circulation grou P〉branch collateral circulation grou P〉branch without collateral circulation group, comparison between groups were statistically significant (P〈0.05). Conclusion:Three dimensional speckle tracking abnormality can be found in left ventricular systolic function in patients with early stage of coronary heart disease. It is valuable to deter-mine the severity and prognosis of coronary heart disease.
